About this file

This document is a Task Assignment Notification Form for Contract HW557 issued by the Florida Department of Environmental Protection (FDEP). The primary parties involved are the FDEP, Arcadis U.S., Inc. (contractor), and the property owner of Bacon's Drive-In Cleaners facility in Haines City, Polk County, Florida. The task assignment was authorized by FDEP Contract Manager Theresa C. Pepe and signed by all parties in October 2020.

Task Assignment DC052B is valued at $82,851.06 and covers a Phase Assessment at the former drycleaning facility. The work comprises three primary subtasks: Project Management (calculated as a percentage of total labor), Site Assessment (soil, vapor, and groundwater screening with monitor well installation), and Site Assessment Report preparation. The deliverables include fieldwork completion by December 31, 2020, and a final Site Assessment Report with Investigative Derived Waste (IDW) manifests due by February 26, 2021. Invoice payments are structured in two tranches: $70,729.22 upon FDEP approval of fieldwork activities, and $12,121.84 upon approval of the Site Assessment Report, IDW manifests, and project management. The work is conducted under a fixed-price agreement with no retainage. Subcontractors selected through competitive bidding include Preferred Drilling Solutions for drilling services ($15,890 combined), Blood Hound for utility location and GPR survey ($1,260), and Clark Environmental for IDW disposal ($3,900). Laboratory analysis through Xenco Analytical and field expenses are budgeted at $25,070 and $3,671.95 respectively, with an anticipated completion timeframe of approximately seven days for fieldwork.

SUBJECT: Task Assignment DC052B (HW557)

This Task Assignment is for the Site Assessment at the former Bacon’s Drive-In Cleaners site.

The fieldwork will be completed in approximately seven days and a Site Assessment Report (SAR) will be submitted as the final deliverable. All work will be performed in accordance with the approved work plan.

The final task assignment total is $82,851.06. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

Subtask 2: Site Assessment This subtask provides funds for soil, vapor and groundwater screening and monitor well installation at the facility. The site is a petroleum solvent site so a mobile lab is not recommended since mobile labs typically cannot analyze all potential contaminants of concern.

FDEP’s geologist approved sampling locations and the overall approach to the assessment presented in the August 2020 updated Work Plan.

Blood Hound will provide a GPR survey & sewer camera video based on their low bid of three bids solicited.

Preferred Drilling Solutions was selected as the drilling subcontractor for DPT work and monitor well installation based on their low bid of three bids solicited.

Subtask 3: Site Assessment Report (SAR) All activities and analytical results obtained during site assessment work will be submitted to FDEP as a written report. This report will include appropriate figures, graphs, data tables, etc.

Additionally, Arcadis will provide electronic deliverables in the approved FDEP format. IDW manifests will also be provided to FDEP as part of the final deliverable.

Initial Site Assessment Work Plan

Arcadis .com C:\Users\Pepe_t\Desktop\Bacons_Cleaners_ISA_WP_03082020 Final_Rev1_08.2020.docx 5 use, verify the presence of utilities, potential drill locations/access, and complete applicable portions of the Site Visit Checklist. It was confirmed that Polk County provides drinking water and sanitary sewer connections for the Site and that no septic systems currently exist. Natural gas, telephone, and electric is also supplied to the Site. No private supply wells were observed on the property. The current Site layout is provided on Figure 3.

The location of the 1996 soil sample could not be verified through on-Site observations or communication with the current property tenant. The location of the former drycleaning machine was unable to be confirmed, as there is now an enclosed walk-in freezer unit running along the eastern boundary of the Site building. The boiler room and rear door referenced in the historical Site Screening Report are no longer present at the Site. The remainder of the interior of the convenience store contains shelves of merchandise with narrow aisles between. A copy of the completed Site Visit Checklist and current Site photographs are presented in Appendix F. Arcadis requested copies of facility as-built drawings from the current owner and has not received them as of the submittal of this work plan.

5 INITIAL SITE ASSESSMENT WORK PLAN

A flexible assessment approach will be used to create a site-specific unsaturated soil and groundwater database capable of determining the absence/presence and extent of drycleaning solvents above soil and groundwater CTLs. In addition, potential risks and exposure pathways will be identified and preliminary recommendations and design provided for remedial action, if justified. The proposed assessment activities will be completed in accordance with FDEP’s Standard Operating Procedures (SOP) (001/01), the site-specific HASP, FDEP’s Considerations for Assessment of Drycleaning Solvent Contaminated Sites (FDEP, 2002), and Conducting Contamination Assessment Work at Drycleaning Sites (SCRD, 2010).

Only fixed-based laboratory samples are recommended since petroleum-based solvents are suspected to have been used and the mobile-laboratory methods would not be able to analyze all the potential contaminants of concern. Also, based on the Site land elevation compared to nearby lake surface water elevations, the assumed depth-to-groundwater is 40 ft BG at this Site; therefore, collection of grab groundwater samples using direct-push technology (DPT) is not recommended. This work plan employs the following methods to complete the initial phase of assessment.

• Utilization of DPT to collect continuous soil samples in the vadose zone (assumed 40-feet thick) and 10 feet into the water table with field screening using a photoionization detector (PID) capable of detecting to parts per billion (ppb), commonly referred to as a ppbRAE® and collection of samples for analysis utilizing a National Environmental Laboratory Accreditation Program (NELAP)-accredited fixed-base laboratory.

• Completion of a deep soil boring at SB005 to approximately 55 ft BG for lithologic characterization.

• Completion of a Modified Active Gas Survey (MAGS) to assess sub-slab soil vapors and potential shallow vadose source zones.

• Installation of permanent monitor wells and collection of groundwater samples for analysis by a NELAP fixed-base laboratory.

C:\Users\Pepe_t\Desktop\Bacons_Cleaners_ISA_WP_03082020 Final_Rev1_08.2020.docx 6

• Surveying the top of casing elevations of monitor wells and collecting water level measurements to confirm groundwater flow direction.

Site assessment methods and techniques are described in detail below.

5.1 Site Preparation and Utility Locate

Arcadis will contact the current Site operator and current property owner representative to share proposed sampling locations and advise schedule/notification requirements prior to completing field work.

Arcadis will then conduct a Site visit to oversee a subsurface private utility locate using ground penetrating radar (GPR) on the Site. The purpose of the utility locate will be to establish the locations of underground utilities (Sunshine State One Call will initially locate utilities on and surrounding the Site).

The initial Site visit indicated the presence of overhead electric, communications, water, sewer, storm water and gas lines as shown on Figure 3. A scan of the sewer lateral will be completed with a video camera to determine potential defects that could have historically provided a source of solvents to the sub-surface.

5.2 Soil Assessment and Sub-Slab Soil-Gas Sampling

5.2.1 Unsaturated Soil Assessment

To confirm if past drycleaning operations at the Site had an adverse impact to unsaturated soil quality conditions, six deep soil borings (SB001-SB006) and two interior shallow soil borings (SB007 and SB008) are proposed for installation at the locations shown on Figure 4. The initial sample location will be placed in vicinity of the confirmed 1996 mineral spirits detection (within the former boiler room) and within the former solvent/diesel AST containment area. Additional boring/sample locations will be placed surrounding the building, within the building interior (shallow borings only), and along the sanitary sewer lateral (determined during GPR). Soil samples from each boring will be described by an on-Site Arcadis geologist. Lithologic logs of the general geology at the Site will be developed from the soil samples collected from the soil borings.

The soil borings will be installed using hand auger and DPT. A concrete coring machine will be used to bore through the concrete floor inside the building to access the underlying soils. Soil samples will be collected at two-foot depth intervals starting at land surface to a depth of 10 ft BG. From 10 to 50 ft BG (assuming water table at 40-ft), screening samples will be collected at 5-ft intervals. Soil samples for field-screening will be transferred to mason jars, covered with aluminum foil and screened with a ppbRAE equipped with a photo-ionization detector to determine if the soils contain the presence of synthetic organic vapors. Additional screening samples may be collected based on field observations.

Soil samples at boring locations in the former solvent tank area and boiler room will have each interval described above collected for fixed-base laboratory analysis (total of 28 samples). For the remaining locations, soil samples will be collected from each boring for analysis by the fixed-base laboratory at the following intervals, unless screening results or field observations identify other impacted soil zones.

Assuming four (4) deep soil borings are installed with four (4) analytical samples collected at each and two shallow soil borings are installed with three (3) analytical samples collected from each, a total of 22 samples for laboratory analysis are planned as summarized below:

C:\Users\Pepe_t\Desktop\Bacons_Cleaners_ISA_WP_03082020 Final_Rev1_08.2020.docx 7

• Shallow: 0.5-ft, 2-ft, and 4-ft.

• Vertical extent below 4 ft BG (at deep borings): One sample from interval exhibiting highest vadose zone screening result or at the water table interface.

A total of 50 soil samples will be submitted to a fixed-based laboratory for analysis of volatile organic compounds (VOCs) by EPA Method 8260, semivolatile organic compounds (SVOCs) using EPA Method 8270, and petroleum range organics using Florida Method FL-PRO. Following the sampling, the boreholes will be sealed with cement grout. This work plan assumes that three (3) days are sufficient to complete the DPT soil assessment.

5.2.2 Modified Active Gas Sampling

MAGS will be completed in general agreement with the Modified Active Gas Sampling Manual, dated October 12, 2004. Nine MAGS wells (MAG01 – MAG09), constructed of 2-inch diameter PVC with 0.010-inch slotted screen set from 0.5 to 5 ft BG, will be installed using a hand auger. The concrete slab inside the building and asphalt pavement surrounding the building appears to provide adequate surface cover seal. Final MAGS well construction will be field determined. One vapor sample will be collected from each MAGS well for analysis of volatile organic compounds (VOCs) by EPA Test Method TO-15. Proposed MAGS well locations are shown on Figure 4.

5.3 Groundwater Assessment

5.3.2 Monitor Well Installation Activities

Arcadis will install four (4) monitor wells at select locations based on soil screening results and potential historical source areas to provide permanent groundwater monitoring locations. Four (4) monitor wells (MW001 – MW004), two-inch in diameter, will be installed to help characterize the source area for potential solvent-impacts and downgradient extent, assuming a groundwater flow to the southwest based on surface topography. For purposes of this work plan, the wells are planned to be constructed of 15-ft of 0.010-inch slot screen, between 35 to 50 ft BG, to bracket the water table assumed to be at 40 ft BG.

Final well locations, dimensions, including depth, screen interval and diameter will be determined based upon the soil DPT results and groundwater elevations encountered during the initial assessment activities. All soil cuttings will be containerized in 55-gallon drums and staged on-Site. This work plan assumes that three (3) field days will be needed to complete the installation and development activities.

5.3.3 Monitor Well Sampling

Arcadis will measure water levels and sample the four newly installed wells for analysis of VOCs by EPA Method 8260, SVOCs using EPA Method 8270, and petroleum range organics using Florida Method FL- PRO. Because of the presumed depth to groundwater, a submersible pump is planned for the sampling activities. All purge water will be containerized in 55-gallon drums and staged on-Site.

C:\Users\Pepe_t\Desktop\Bacons_Cleaners_ISA_WP_03082020 Final_Rev1_08.2020.docx 8

5.4 Well Location/Elevation Survey and Area Reconnaissance

The monitor well top of casing elevations will be surveyed by Arcadis field personnel relative to an arbitrary point to the nearest 0.01-ft to determine current groundwater flow direction. The horizontal locations of soil borings, MAGS points, and monitor wells will be field measured from the building or surveyed using GPS.

Arcadis will attempt to identify any changes in reported current land use, potential future land use, and potential exposure pathways (direct and indirect) on or near the Site. Additionally, Arcadis will perform a windshield survey of public and private supply wells within one-quarter mile of the Site.

6 SITE ASSESSMENT REPORT

A draft SAR that describes the results of the proposed site assessment tasks described herein will be submitted to the FDEP within 60 days of completion of the field work. The SAR will contain tables and figures summarizing the results of assessment activities and include conclusions and recommendations based upon findings. A draft SAR will be submitted to the FDEP for review and comment. A final SAR will be submitted to the FDEP within two weeks of receipt of FDEP comments.

7 INVESTIGATIVE DERIVED WASTE

Investigative derived wastes (IDW - drill cuttings and purge water) generated during site assessment activities will be drummed, labeled and temporarily staged on-Site in Department of Transportation (DOT)-approved, 55-gallon steel drums. Arcadis will coordinate with a waste disposal contractor for non-hazardous disposal of the IDW and provide copies of final disposal manifests to the FDEP.

8 QUALITY ASSURANCE & QUALITY CONTROL

All fieldwork will be performed in accordance with FDEP’s SOP (001/01) and the site-specific HASP included under separate cover. Additionally, sample analyses will be performed by NELAP-certified laboratories to method practical quantitation limits. Appropriate QA/QC samples will be collected during the assessment.

9 PROJECT SCHEDULE AND COST

Cost estimate/level of effort (CEF-LOE) worksheets and subcontractor bid copies for the implementation of this work plan are provided separately. The proposed resource loaded schedule for the site assessment and SAR document package, is presented in Appendix G.
