Proposal for Groundwater Monitoring at Kathy’s Cleaners, 202 West Howard Street, Live Oak, Florida, FDEP Facility Identification Number ERIC_5504
Dear Ms. Pepe:
Arcadis U.S., Inc. (Arcadis) is pleased to submit this level of effort (LOE) and cost proposal to conduct annual groundwater monitoring at the Kathy’s Cleaners Facility (“the Site”). This proposal includes one annual groundwater monitoring event and one letter report. The FDEP cost estimating spreadsheets and schedule are attached.
SCOPE OR WORK
In accordance with Chapter 62-780 of the Florida Administrative Code (FAC), and based on recommendations in the most recent Groundwater Monitoring Letter Report and discussions with FDEP, Arcadis propose the following:
Subtask 1 - Project Management
Subtask 1 is reserved for project management as specified by FDEP and is calculated automatically in the cost estimating form.
Subtask 2 - Groundwater Monitoring
ARCADIS will conduct one annual groundwater monitoring event. ARCADIS will collect groundwater samples from nine monitoring wells (MW001, MW002, MW005, MW008, MW009, and MW011 through MW014) and one equipment blank in accordance with the FDEP SOPs. Each sample will be analyzed for VOHs. Monitoring well purge water will be treated and disposed of onsite.
