About this file
This document is Amendment No. 1 to DEP Contract No. HW557, a contract amendment executed on May 14, 2011, between the Florida Department of Environmental Protection (FDEP) and ARCADIS U.S., INC. The amendment was authorized by Teresa Boeshaghi, DEP Contract Manager (MS4505), and includes a Separation of Duties Notice dated April 18, 2011, prepared by Contracts Office Representative Jim Reynolds.
The amendment increases the contract's funding increment from $705,000.00 to $915,000.00, representing an increase of $210,000.00. The amendment adds employment eligibility verification requirements, mandating that the contractor enroll and participate in the federal E-Verify Program for Employment Verification within thirty days of the amendment's effective date, with documentation provided in the form of an E-Verify "Edit Company Profile" screen. The contractor must also require all subcontractors performing work under the contract to enroll in E-Verify within ninety days of the effective date or within ninety days of entering into a subcontract, whichever is later. The contractor must maintain records of participation and compliance with E-Verify provisions and make such records available to the Department or other authorized state entities. Compliance with the Employment Eligibility Verification provision is designated as an express condition of the contract, with non-compliance treated as a material breach.
