RE: CN802 Task Assignment #5 – Change Order #1 Ft. Clinch Road Repair & Waterline Replacement
Dear Meena, For your review, CPH Consulting LLC, has provided a proposal summary and man-hour documentation for the services tasks identified herein. This proposal deals with the re-routing of the water line replacement along approximately 3,998 LF. Based on our discussion CPH used the rates from contract CN210 for all tasks included in this Change Order. It is assumed the previously paid reimbursable cost for the FFWCC permit fee shall be transferred to the modified application for the new water line route.
Please review the attached Proposal Summary and Deliverables.
Sincerely, CPH Consulting, LLC
Amy Daly, LEED AP Director of Environmental Services g, Ft. Clinch State Park Ft. Clinch Road Repair & Waterline Replacement
CN802 Task Assignment #5 Change Order #1
An asterisk “*” signifies the task is priced using rates approved under CN210 contract at the request of FDEP.
Subtask 1: Gopher Tortoise Survey and Permitting Subtask 1A is canceled and new subtasks 1D through 1I are added
Subtask 1D: Gopher Tortoise Survey Permitting Current Route*
CPH shall prepare and submit documentation, photographs and photograph location map and graphics to FFWCC to process a Temporary Exclusion Permit Application for on- site relocation.
FDEP Project Manager shall identify the FDEP representative signing the FFWCC permit, provide their contact information and the FDEP FEIN# for purposes of completing the FFWCC permit application.
CPH will prepare additional information requested by FFWCC during the permitting to process the application.
CPH will provide the permittee the permit issued by FFWCC for signature.
Subtask 1E: Gopher Tortoise Survey – New Route* Description:
CPH will conduct a 100% field survey covering the new project corridor for the water line and resurvey the recipient site within the park to the east of the waterline project area. The survey will investigate suitable tortoise habitat to locate gopher tortoise burrows and identify the burrow status & marking in the field within the project site.
CPH shall locate the approximate burrow location on an aerial photograph as required by FFWCC with GPS coordinates.
The survey shall be conducted by a CPH scientist holding a FFWCC Authorized Gopher Tortoise Agent permit.
CPH shall prepare a graphic documenting the field survey results identifying the approximate burrow location, status and coordinates. The survey is valid for 90 days pursuant to FFWCC permitting requirements.
CPH shall document the project location and on-site relocation areas with photographs depicting existing conditions for use in FFWCC application submittal.
CPH recommends conducting the 100% survey of the project corridor and two on-site relocation areas at least 90 days prior to the start of construction.
Subtask 1F – Gopher Tortoise FFWCC Permitting- Application Modification for the New Route*
CPH shall modify the application information submitted to FFWCC.
CPH shall prepare and submit documentation, photographs and photograph location map and graphics to
FFWCC to process a Temporary Exclusion Permit Application for on- site relocation.
CPH will prepare additional information requested by FFWCC during the permitting to process the application.
CPH will provide the permittee the permit issued by FFWCC for signature. FDEP shall provide the sign application back to CPH.
Subtask 1G – Gopher Tortoise Bucket Trap Installation * Description: CPH will coordinate with FFWCC regarding required noticing as specified in the tortoise relocation permit prior to bucket trap installation. CPH shall install up to fifteen (15) bucket traps within the project site. FDEP shall also identify at least 2 assistants (Park staff) to assist with bucket trapping requirements throughout the 28-day trapping period and provide their contact information to CPH. CPH shall prepare a written authorization to Park staff to act as Permit Assistants for the bucket trapping permit requirements. CPH shall meet with Park staff to review FFWCC bucket trapping protocol.
Subtask 1H: Gopher Tortoise Bucket Trapping* Description: CPH shall create a schedule for bucket trap checking for the 28-day period for distribution to the two (2) FDEP Permit Assistants. CPH shall coordinate with Park staff to act as Permit Assistants for at a minimum one (1) daily check of the installed bucket traps and maintenance of bucket traps. Park staff shall check the traps for up to 28 days.
If tortoises are captured, Park staff shall relocate the tortoises to the on-site recipient site designed relocation area after collection of morphological data. Prior to release within the State Park, each relocated tortoise shall be sexed (adults only), measured and permanently and uniquely marked by scute-notching per established FFWCC guidelines and permit requirements. CPH will use the on-site recipient site previously identified by FDEP. CPH on the 29th day shall check the traps and remove the bucket trap and provide the interim After-Action Report to FFWCC for their approval.
Capturing tortoise via supervision by backhoe excavation is not included in this scope but can be provided as an additional service if deemed necessary. CPH assumes the 28-day bucket trapping period shall not be interrupted by cold weather temperatures which stop the trapping period. If the cold weather affects the bucket trapping 28-day consecutive monitoring period, CPH shall submit additional services for approval.
Subtask 1I: Gopher Tortoise Final After Action Report* Description: CPH shall coordinate with the contractor and Park staff during the period after initial relocation efforts and final submittal of the After-Action Report. After project construction is complete or 5 days prior to permit expiration, whichever occurs first, CPH shall submit the Final After Action Report to FFWCC for approval.
Gopher Tortoise Scope Exclusions or Tasks Provided by Others
The FDEP shall identify the FDEP representative signing the FFWCC permit and their contact and provide the FDEP FEIN# for purposes of completing the FFWCC permit application.
FDEP is responsible for the payment of all permit application fees, mitigation contributions, and recipient fees associated with permitting and relocation of tortoises.
The FDEP selected contactor shall be responsible for installing exclusion fencing.
FDEP shall be responsible for identifying two (2) potential on-site relocation areas.
Capturing tortoise via supervision by backhoe excavation, rod pulling and others means are not included in this scope but can be provided as and additional service if deemed necessary.

Universal Engineering Sciences will be the subconsultant to perform the geotechnical engineering explorations. The scope of work will include: 1) Complete field work and laboratory testing on two Standard Penetration Test (SPT) borings (one within each pit area) to depths of 10 feet. Based on site observations, due to the topography, it appears that the borings will require the use of UES portable tripod equipment. Due to the nature of the equipment required to perform the test borings, some property disturbance should be expected. This proposal does include limited site clean-up including backfilling the boreholes with soil-cuttings for safety considerations. No other restoration services (i.e., pressure washing, landscaping, repairing wheel ruts, etc.) are included in this proposal. Universal understands that the rights of entry and access to the property will be provided prior to and at commencement of field activities. 2) Provide the results of the above exploration and engineering study in a signed report.
