We propose to hereby furnish the following:
Our Proposal includes Labor, Material, Equipment, and Workmanship necessary to perform all work on the Lake Larc Dam as requested by Comak Group.
Mobilization 1 - An exploratory dive will be performed on the 54" riser to verify the integrity of the dam along with the condition of the gate valve. A day rate will apply and time will be allocated to troubleshooting the blockage of Lake Larc if the valve is froze up or broken. APS is not responsible for the repair of the valve if it is found to be broken.
Mobilization 2 - If the gate valve is in working condition it will be closed, at that time we will plug the lower end isolating the 24" culvert. We will dewater that culvert before performing an internal pipeline inspection with our mainline camera along with a video inspection of the 54" riser pipe. We will review the condition of this pipeline/riser and email a link to the video and report within 48 hours.
APS is not responsible to the condition of the Lake Larc Dam, before during or after inspection.
