About this file
This is a Quality Assurance Surveillance Plan (QASP) for the USS RAFAEL PERALTA (DDG-115) FY26 6J1 SIA Farm Out Bundle-1 contract, solicitation number N6264926RA036. The QASP serves as Attachment VII to the contract and establishes the Government's performance criteria, standards, and procedures for monitoring contractor deliverables to ensure they are timely, adequate, and complete while meeting all contract requirements.
The plan designates the Administrative Contracting Officer (ACO) as ultimately responsible for determining the adequacy of contractor performance and ensuring compliance with contract terms, while the Project Manager/Contracting Officer Representative (PM/COR) handles technical administration and maintains a quality assurance file. Surveillance methods include direct on-site observation, verification of documentation and deliverables for accuracy and completeness, and inspections as needed. The Government will assign ratings of either acceptable or unacceptable based on performance standards outlined in Enclosure (1), with continuous monitoring and periodic analysis of surveillance frequency. Enclosure (1) identifies five key performance areas: (1) Schedule and Associated Reports per NAVSEA Standard Item (NSI) 009-60, requiring submission of Initial Integrated Production Schedule, Schedule of Record, Weekly IPS, and 25/50/75 percent conference support documents; (2) Corrective Action Requests (CARs) requiring timely responses; (3) Condition Found Reports (CFRs) submitted to SRF-JRMC per contract specifications; (4) Letters of Concern (LOCs) and Letters of Direction (LODs) issued by the ACO; and (5) Cure Notices and Show Cause Notices per FAR 49.402-3. Contractor responses must be complete and adequate by specified due dates, and all performance information will be documented for use in Contractor Performance Assessment Report System (CPARS) records.
