About this file
This is a Past Performance Evaluation Form prepared by the District of Columbia Department of General Services for the Design-Build Services for Hendley ES Modernization project. The form is used to assess contractor performance across five key evaluation categories: Quality of Services/Work, Timeliness of Performance, Cost Control, Business Relations, and Customer Satisfaction. Evaluators are required to rate contractor performance in each area using a six-point scale ranging from 0 (Unacceptable) to 4 (Excellent), with a ++ (Plus) designation also available. The form requests evaluator contact information, identification of the service type received, contract number, contract amount, period of performance, and detailed remarks supporting ratings for both excellent and unacceptable performance.
The rating guidelines provide specific evaluation criteria for each performance category. Quality assessment focuses on compliance with contract requirements, accuracy of reports, technical excellence, and responsiveness to technical directions. Timeliness evaluation measures achievement of interim milestones, on-time completion including wrap-up, prompt notification of problems, and absence of liquidated damages. Cost control examines budget compliance, cost efficiency, appropriate personnel costs, and accurate billing. Business Relations assesses reliability, effective management, businesslike correspondence, and responsiveness to contract administration. Customer Satisfaction evaluates the contractor's responsiveness to inquiries and technical/service/administrative issues. Each rating level—from Unacceptable through Excellent—is defined by the degree to which nonconformances, cost issues, delays, or response deficiencies impact achievement of overall contract requirements and consumption of agency resources.
