About this file

This is a Past Performance Questionnaire template designed to collect references from the Court Services and Offender Supervision Agency (CSOSA) regarding a contractor's prior experience delivering personal property management systems. Completed questionnaires must be submitted by August 4, 2026 at 11:59 PM Eastern Time to catherine.collins@csosa.gov. The questionnaire requests information about a reference contract where the contractor provided supplies or services similar to the requirements in solicitation 9594CS26Q0025, which seeks a Personal Property Management System as a Software as a Service (SaaS) solution with FedRAMP Moderate Authorization and maintenance/support services.

The questionnaire gathers specific performance data from the most knowledgeable individual within the reference organization, including contract period, total value, and whether the respondent's organization received the services as a prime contractor, subcontractor, or key person. Respondents rate the contractor's performance across twelve detailed criteria using a six-point scale (Exceptional to Neutral), organized into three categories: Technical Performance & System Quality (asset tracking accuracy, reporting capabilities, system stability, and user intuitiveness); Data Migration, System Integration, Training & Support (legacy data transition, configuration and integration, patch deployment, training quality, and help desk responsiveness); and Project Management & Business Relations (schedule adherence, proactiveness in problem-solving, technical expertise availability, and flexibility with changing requirements). Respondents must provide narrative explanations for any ratings of 1, 2, or 3, and are encouraged to supplement responses with observations from others within their organization.
