About this file
This is a Past Performance Questionnaire (PPQ) template from the Transportation Security Administration's (TSA) Screening Partnership Program, to be completed by references for contractors. The questionnaire includes sections for basic contract information (contract number, value, role) and detailed performance ratings across six major categories: Quality of Service, Schedule, Cost, Management, Small Business Subcontracting, and Regulatory Compliance.
The rating scale ranges from Exceptional to Unsatisfactory, with specific performance definitions provided for each level. Respondents must evaluate the contractor's performance on various aspects including conformance to requirements, timeliness, cost control, management effectiveness, communication, and compliance with contract terms. The form requires explanatory comments for any ratings below Satisfactory and must be submitted to TSA Contract Specialist DeAndre Rawlings. This template is part of RFP 70T05025R5900N002 for airport screening services under TSA's Screening Partnership Program.
