About this file
This is a Past Performance Questionnaire template designed to evaluate contractor performance on previously completed federal contracts. The form requests identification of the company name, contract number, type of work performed, contract period, and estimated contract award amount. Respondents are instructed to rate contractor performance on a five-point scale ranging from unacceptable (1) to superior performance (5), with options to mark items as not applicable (NA).
The questionnaire assesses seven key performance areas: compliance with contractual terms and technical requirements, reasonableness and cooperation in resolving customer requests and complaints, flexibility in meeting customer requirements, adherence to contractual response times, invoice clarity and accuracy, overall performance, and likelihood of awarding future contracts to the same company. Each evaluation item includes space for comments to provide context or substantiation for the ratings selected. This template is intended to be completed by government representatives or contracting officers who have direct knowledge of a contractor's performance on the referenced contract.
