About this file

This document contains a past performance questionnaire template for a federal construction contract opportunity. The past performance questionnaire requests information on a contractor's previous projects to evaluate their ability to complete the EHRM Infrastructure Upgrades Construction project for the Department of Veterans Affairs Technology Acquisition Center Austin. The questionnaire asks the contractor to provide details of relevant prior contracts, including quality, schedule, communication, management, cost, and subcontract management, and allows a client reference to rate performance in these areas. It also collects information on the client reference, their role, and an overall recommendation on whether they would work with the contractor again.
