About this file
This is an Insurance Certifications attachment for a Request for Offers (RFO) issued by the California Department of Aging for the MSSP - Organizational Change Management contract (RFO-2526-04). The document outlines the insurance requirements and certification obligations that offerors must acknowledge and comply with if awarded an agreement. Offerors are required to certify their understanding of the insurance requirements detailed in Attachment 18 (Sample Agreement, Exhibit E) and confirm their ability to provide all required insurance documentation upon contract award, including certificates of insurance and proof that the State of California, its officers, employees, and agents are properly insured. The document specifies that proof of insurance must be submitted by awarded firms and that the California Department of Aging reserves the right to cancel a proposed award to any offeror who fails to submit required documentation in a timely manner.
The attachment provides detailed guidance on ACORD Certificate endorsement requirements, including specifications for the insured name, insurance types and limits, certificate holder information, and policy expiration dates. All required insurance types must remain active and unexpired, with awarded firms advised to submit updated certificates upon policy renewal. The State of California, its officers, employees, and agents must be listed as additional insured parties through either an endorsement to the offeror's policy or a copy of the current insurance policy demonstrating existing coverage. For offerors utilizing sub-offerors to complete portions of the agreement, Commercial General Liability insurance must either include the sub-offeror as an additional named insured or require each sub-offeror to maintain their own policy. Importantly, offerors are not required to submit proof of insurance with their initial offer; proof is only required following contract award.

Initial Certification Offeror acknowledges that they have reviewed and understand the insurance requirements for this request as described in Attachment 18, Sample Agreement, Exhibit E, Insurance Requirements.
Offeror acknowledges that they have reviewed and understand the sample insurance forms provided with this attachment.

Certificates of insurance showing the offeror’s insurance coverage types and limitations.
Proof that the offeror has insured the State of California, its officers, employees, and agents. Offeror further understands that this proof shall come in the form of either:
1. An endorsement to the offeror’s insurance policy (Form CG 20 10 11 85 or as broad as),
2. A copy of the offeror’s current insurance policy that shows that the policy already insures all parties required to be insured by this Agreement.
Offeror certifies that, should Commercial General Liability insurance be required, and should the offeror use a sub-offeror to complete a portion of this Agreement, the offeror shall either:
1. Include the sub-offeror as an additional named insured under the offeror’s policy, or
2. Ensure that each sub-offeror is insured under their own Commercial General Liability policy.
Offeror acknowledges that they understand that the CDA reserves the right, at its sole discretion, to cancel a proposed award to an offeror who does not submit the required documentation described herein.
