About this file

This is a Commercially Useful Function (CUF) Certification form issued by the California Department of Aging for RFO-2526-04, an MSSP Organizational Change Management procurement. The form must be completed and submitted by all new and renewing Small Business (SB) and Disabled Veteran Business Enterprise (DVBE) applicants as part of their proposal response. The certification verifies that SB, Microbusiness, and DVBE contractors, subcontractors, and suppliers meet the commercially useful function requirements under California Government Code 14837(d)(4) for Small Businesses and Military and Veterans Code 999(b)(5)(B) for DVBEs.

The form requires respondents to certify that the SB or DVBE entity will provide goods and/or services as distinct elements of the contract work, that the work is normal for the company's business operations, and that the company will perform, manage, and supervise the work as committed. For contracts involving goods, the respondent must confirm responsibility for negotiating price, determining quality and quantity, ordering, making payment, and installation if applicable. For prime contractors performing all work, compliance is determined directly. For subcontractors or primes utilizing subcontractors, respondents must document that subcontracting percentages align with industry standards and provide written statements detailing the subcontractor's role, services, and goods. The form requires respondent signature and certification that all stated requirements have been satisfied.
