About this file
This is a Respondent's Rate Sheet attachment for the California Department of Aging's RFO-2526-04 for Managed Support Services Program (MSSP) Organizational Change Management services. Respondents must submit hourly labor rates for organizational change management services as detailed in the Scope of Work, with rates capped at those listed in the TDDC Master Service Agreement. The offeror's final submission constitutes an irrevocable offer valid for 180 calendar days from submission, and CDA reserves the right to adjust or omit portions of work as deemed necessary. The contract term rates and all associated expenses including labor, materials, licenses, permits, transportation, and travel per diem must be included in the quoted rates. Respondents are required to use the provided Excel template to clearly define budget categories and tie each task in the Scope of Work to corresponding budget items and technical approach details. All payments are made monthly in arrears.
Pricing must be submitted using the provided Excel spreadsheet template with all line items completed; responses submitting "N/A" or blank dollar amounts will be rejected, and zero-dollar amounts indicate the firm will perform those services at no cost to the State. Respondents must guarantee that billing rates will not increase during the contract term and must not be reimbursed for travel-related expenses, which must be factored into quoted rates. Unanticipated costs not specified in the Scope of Work are capped at five percent of the total Base Contract Term value. If subcontractors are used, their roles and reimbursement methods must be clearly identified in both the technical offer and budget. Small Business Preference and/or Disabled Veteran Business Enterprise Incentive points, if applicable, will be applied after cost calculations are completed.
