About this file
This is a contractor experience questionnaire template (Attachment 5) designed to collect detailed information about bidders' past performance on fire protection control systems projects. The questionnaire requires contractors to provide specific details about previous projects including project names, customers, locations, types of systems installed/maintained, equipment brands used, facility sizes, system complexity levels, and applicable governing criteria/standards.
The template includes dropdown menus for standardized responses covering project types (site assessment, maintenance, design, installation), system types (fire detection, mass notification, suppression systems), equipment brands (ADT, Edwards, Notifier, etc.), and governing standards (UFC and NFPA specifications). It also specifically captures Risk Management Framework (RMF) experience, period of performance dates, prime/sub status, and facility security clearance requirements. The questionnaire appears to support the broader DLA Enterprise Fire Protection Control Systems (FPCS) solicitation by helping evaluate bidders' relevant experience across various fire protection system types and complexity levels.
