About this file
This is a Past Performance Questionnaire (PPQ) template designed for offerors responding to the Nutrition Care Food Service solicitation (HT940827RE003) issued by the Defense Health Agency. The PPQ serves as an evaluation tool to assess contractor past performance and must be completed in two parts: Part 1 is completed by the offeror with contract details including offeror name, contract number, award date, completion date, scope of services, and total contract value; Part 2 is completed by the customer (the contracting officer or representative with first-hand knowledge) who evaluates the contractor's performance across three subfactors—Quality, Schedule, and Management—using a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). The completed and customer-signed PPQ must be submitted as a PDF document in Volume IV of the proposal.
The questionnaire captures whether the contractor performed as a prime or subcontractor and identifies the customer type (Commercial, Federal Government, State Government, or Other). For each performance subfactor, the customer provides qualitative ratings and optional comments regarding compliments, problems, and corrective actions taken. This template does not contain specific response dates, pricing terms, or contract award information; rather, it is an administrative appendix designed to standardize the collection of past performance data from previous customers to support the Defense Health Agency's evaluation of offerors for the Nutrition Care Food Service contract supporting five military medical treatment facilities.
