About this file

This document is Standard Form 94 (SF 94), a Statement of Witness form prescribed by the General Services Administration for documenting witness accounts of motor vehicle accidents involving federal government vehicles or privately owned vehicles.

The form collects comprehensive accident information through structured sections including witness identification (name, address, contact information), accident details (date, time, location), and narrative descriptions of how the accident occurred. Witnesses are required to describe apparent damage to both private and government property, road and environmental conditions that may have influenced the accident, and any unusual circumstances observed before or during the incident. The form includes a diagram section where witnesses indicate vehicle positions, directions of travel, pedestrian locations, and other relevant accident scene details using standardized notation conventions. Additional information requested covers the number of vehicles and drivers involved, passenger counts, vehicle descriptions, and whether police, fire, or rescue personnel responded to the scene. The form is subject to the Privacy Act of 1974, with information collected under authority of 40 U.S.C. § 491 and 31 U.S.C. § 7701. The estimated completion time is 20 minutes, and the OMB Control Number 3090-0118 is valid through December 31, 2023.
