About this file
The document is a Past Performance Questionnaire (PPQ) for NASA Langley Research Center's Joint Safety Health Institutional Facilities Team (JSHIFT) solicitation (RFP No. 80LaRC25R7001). The PPQ is designed to evaluate a contractor's past performance across five key performance work statement areas: 1) safety inspections and audits, 2) test operations safety support, 3) fire protection program, 4) comprehensive industrial hygiene program, and 5) radiation protection support. The questionnaire uses a rating system ranging from Exceptional to Unsatisfactory for performance and amount of work performed, with evaluators asked to provide detailed assessments of the contractor's capabilities in each area.
The questionnaire requires detailed information about the contractor, including company details, contract type, performance period, and contract value. Evaluators are instructed to rate the contractor's performance in general areas such as meeting schedules, adaptability, personnel management, and business relations. The document emphasizes that only performance within the past five years is relevant, and the completed questionnaire is considered Source Selection Information. The PPQ is to be sent directly to the NASA Contracting Officer and will be used as part of the source selection process for the JSHIFT solicitation, which is an 8(a)-set-aside competition with an anticipated contract award date of August 15, 2025.
