About this file
This is a Corrective Action Request (CAR) Form template used in federal contract management to document and track quality or compliance deficiencies.
Section I, completed by the government, captures the problem identification and documentation. It requires a tracking number, contract number, CAR severity level (I, II, III, or IV), identification and issuance dates, the contractor site lead receiving the CAR, and a 45-day response deadline from the issue date. The government must document the problem description, supporting evidence, the contractual or performance requirement that was not met, and a statement of nonconformity. This section also indicates whether a Root Cause Analysis (RCA) is required, noting that RCAs may not be required for Level I CARs.
Section II is completed by the affected contractor if an RCA is required. The contractor must identify the root cause of the problem, specify whether corrective action is needed, document the immediate correction with completion dates, and describe corrective actions to prevent recurrence with an estimated completion date. The Site Lead confirms completion once actions are finished. Section III, completed by the initiating government official, evaluates the effectiveness of the contractor's corrective actions, reviews the RCA and proposed prevention measures, and determines whether the Corrective Action Plan is acceptable. The final closure requires signatures from the Unit GFR/AGFR and COR, with notification to PMA-266 GFR prior to formal closeout.
