1. PRINCIPAL FEATURES: The work covered by these specifications and accompanying drawings consists of furnishing all plant, labor, materials, tools, and equipment necessary in performing all operations in connection with Project FXBM 15-1077, DESIGN REPAIR INDUSTRIAL FACILITY 3015.

2. SCOPE: In general, the principal features of the work are outlined but not limited to this paragraph. Unless otherwise indicated, it is not the intent of this listing to include technical data for materials or the installation of these materials. This project consists of rehabilitating the existing industrial waste pretreatment facility (FAC 3015) and the construction of a dewatering building adjacent to FAC 3015. A smaller portion of the project will consist of site improvements in and around FAC 3015 including grading, utility work, and asphalt concrete pavement, and other incidental work.

3. FIELD MEASUREMENTS: The Contractor shall verify all measurements at the site and shall be responsible for dimensions and proper installation of all materials.

4. AVAILABILITY OF UTILITIES: All reasonable quantities of water, natural gas, and electricity will be made available to the contractor without charge. Connections to the utilities are the contractor’s responsibility and shall be coordinated with, and approved by, the appropriate Civil Engineering utility

shop. The water fill stand located north of facility 7510 may be used for filling tank trucks. If connections are made to base fire hydrants, the contractor shall notify the base fire department and install a backflow prevention device as part of the connection.

5. CONTINUED USE OF FACILITIES: All contractor operations must be closely coordinated with the using agency. The Contractor shall not start work before 6:30 a.m. each morning. All utilities shall remain in operation during construction unless specifically noted otherwise.

6. SEQUENCE OF WORK: NOT USED.

7. DISPOSAL OF MATERIALS: Except for excess soil, all materials removed under this contract shall be disposed of off base in such a manner that meets federal, state and local environmental codes.

Excess soil will be delivered, by the contractor, to a storage area within the limits of Ellsworth Air Force Base. The area will be pre-designated by the government and marked by the contractor. At the conclusion of use, the area shall be graded for drainage and re-seeded to establish ground cover.

8. CONTRACTOR SUBMITTALS: Submittals are to be signed by the contractor and provided in accordance with the requirements of other portions of the contract and the Submittal Schedule.

9. CONSTRUCTION PERMITS

9.1 Digging Permits: The contractor will be responsible for coordinating a government supplied, Base Civil Engineer Work Clearance Request (AF Form 103) prior to performing digging of any type. The contractor shall process the digging permit by coordinating with and obtaining signatures from responsible representatives of the organizations listed on the AF Form 103 prior to obtaining final approval from the Air Force Base Civil Engineer or his approved representative. The contractor will be given assistance, by the government, in the execution of the initial Work Clearance Request. Thereafter, government assistance will be limited to an as-needed basis in the event of unusual circumstances. It will be the contractor's responsibility to coordinate completion of the necessary AF Form 103 and arrange to have existing utilities located as indicated on the completed form, prior to the beginning of digging operations in the individual areas. This coordination is anticipated to take approximately five (5) working days to complete per request, and may require coordination with as many as twenty (20) individuals located on or near the base. A copy of the AF Form 103 is attached for reference. Any unusual delay in obtaining approval from any particular organization will be reported immediately to the contracting officer’s representative for assistance.

9.2 Utility Staking Requirements: The contractor shall layout and mark his intended area(s) of excavation before calling for field coordination by utility personnel. This shall be done a minimum of five working days in advance of when digging is expected to begin. Once all responsible utility representatives have field located crossover and/or interference points between the proposed work and existing utilities, and signed off on the digging permit to signify completion of the field coordination of the digging permit, then digging in the area represented by the digging permit may begin. Any utility service markers or markings established by the utility representatives must be maintained by the contractor through the completion of the digging operations. As soon as work requiring the markers is completed, all above-ground marking devices (flags / stakes / etc) shall be removed by the contractor.

9.3 Digging Operations: Digging near established interference or crossover points shall be done by hand, five (5) feet either side of the point along the intended route, in order to prevent disturbing the existing utility. If the existing utility is uncovered in the new excavation, it shall be protected from damage

and movement while in the open excavation and during backfill. The contractor shall be responsible for the repairs and associated costs for repairs of any utility damaged by construction, whose location was made known to the contractor.

10. LAYOUT OF PROJECT: All surveying and layout work is the responsibility of the contractor. The government will assist the contractor only in providing elevations or location to begin the work.

11. SAMPLING AND TESTING: All quality control sampling and testing shall be the responsibility of the contractor. Sampling and testing shall be performed by an approved commercial testing laboratory.

Copies of all test results shall be submitted to the contracting officer.

12. IDENTIFICATION OF EMPLOYEES

12.1 Entry Authority List (EAL): These lists are kept at the gates to verify employment. The following guidelines apply:

12.1.1 On Base: Original EAL and two (2) copies to the Contracting Office is required

12.1.2 The EAL must be in alphabetical order, on company letterhead, containing the complete address and phone number of the company, signed by an authorized company representative, and in the following format:

12.1.3 DO NOT combine your EAL with the subcontractor's EAL. Each company/subcontractor must have a separate EAL.

12.1.4 Additions/Deletions of Personnel: When personnel are to be added or deleted, the entire EAL shall be re-accomplished and submitted to the Contracting Officer.

12.1.5 Escorts: An asterisk (*) on the EAL by key personnel such as a foreman or supervisor will allow temporary entry of supply deliveries / pickup / repair. The escorts must accompany these personnel the duration of time on the installation. A maximum of three (3) key personnel per contractor is allowed. A phone number where the escorts can be reached must be included on the EAL.

12.1.6 Two (2) days are required for processing of EALs.

12.2 Air Force Form 75, Visitors Pass (AF 75): The AF 75 is required for all employees and vehicles, The AF 75 has two parts, one for placement on the driver's side of the dashboard (must be visible at all times), the other part is issued to the individual and must be on his/her person at all times. Each employee must obtain this pass by presenting his drivers license at the Visitor Control Center, located outside the Liberty Gate. If any of the above identification is not on the employee’s person, he/she will be escorted from the base. It is recommended that a magnetic sign be placed on the left door of the vehicles that do not have a company logo. Periodic spot checks of driver’s licenses and AF 75's will be conducted on a random basis.

13. CONTRACTOR’S STORAGE AREA: The contractor will be authorized the use of an area adequate for his materials and equipment storage near the site as determined by the contracting officer at the

preconstruction meeting. The contractor shall set up and maintain his operation within the area in an orderly manner and upon completion of the project, shall restore the premises to its original condition free of all excess materials, debris and excavations or embankments resulting from his operations. The contractor will also be responsible for his own equipment security.

14. TEMPORARY STAGING AREA: The contractor may be allowed to utilize certain locations within the project area as approved by the contracting officer to park equipment and store construction materials.

Staging areas shall be maintained in a neat, orderly manner including erection of temporary fencing around the areas and restored after use.

15. CONTRACTOR'S HAUL ROUTES: Access to the project will be by Bismark (Commercial) Gate during the week (Monday through Friday), and Liberty (Main) Gate during weekends (Saturday and Sunday), thence over the haul route indicated on the Location Plan of the drawings. Work during weekends and holidays will only be allowed with the approval of the contracting officer or his designated representative. The designated routes shall be used by both loaded and unloaded vehicles.

16. TEMPORARY TOILETS: Unless otherwise directed by the contract documents, the contractor shall provide and maintain sanitary temporary toilets for all work accomplished under the project. These toilets shall be located as directed by the contracting officer. The temporary toilets shall be portable, enclosed and weatherproofed and kept in a sanitary condition at all times.

17. CONSTRUCTION SIGNAGE: The contractor shall furnish, install and maintain traffic control signage for street and highway construction operations per the requirements of the Manual on Uniform Traffic Control Devices for Street and Highways. Prior to the contractor beginning construction, a detailed traffic control plan showing all construction signage shall be submitted to the contracting officer for review and comment.

18. EXISTING CONDITIONS: The plans represent the best available information at the time they were prepared. While every effort has been made to correctly show all major existing items which will affect the work, it is recommended that prospective bidders examine the site prior to submitting a bid. No additional payment will be made for extra work required by obstacles which were in place and clearly visible at the time of bidding.

19. WORK SCHEDULE: Working hours for the contractor will normally be between the hours of 6:30 a.m. and 5:00 p.m. excluding Saturdays, Sundays and federal holidays. If the contractor desires to work during periods other than above, additional government inspection forces may be required. The contractor must make his / her request to the contracting officer three days in advance of his / her intention to work during other periods to allow assignment of additional inspection personnel. The contracting officer shall determine, at his discretion, whether such inspection forces are available and whether work will be allowed on a non-scheduled day. The contracting officer may authorize the contractor to perform work during periods other than normal duty hours / days; however, if inspectors are required to perform in excess of their normal duty hours / days solely for the benefit of the contractor, the actual cost of inspection at overtime rates will be charged to the contractor. These adjustments to the contract price may be made periodically as directed by the contracting officer.

20. DAILY CLEAN-UP: The contractor shall at all time keep the premises free from accumulations of waste materials or rubbish caused by his employees or work. At completion of each work day, he shall remove all rubbish from the construction site. All his tools, equipment, and surplus materials, etc., shall be

stored in the storage area or temporary staging area.

21. CONTRACTOR DAMAGES: Areas not identified to be removed / worked under this contract that are disturbed or damaged during construction shall be removed and replaced at no additional cost to the Government.

22. UTILITY ADJUSTMENTS: The contractor shall adjust or shall arrange to have adjusted, utilities that are in conflict with new construction. The contractor shall be responsible for adjusting any water, gas, communication line, and electrical lines. The contractor shall employ qualified personnel to perform these adjustments.

23. UTILITY OUTAGES: The contractor shall perform the work under this contract with a minimum of outage time for all utilities. If possible, the contractor shall schedule any work that requires utility outages (electrical, water, gas, etc.) which will close down or limit normal activities in the building, construction area, or other affected areas at a time other than regular work periods of the organization occupying the facility. The contractor shall be responsible for coordination of all utility outages executed for this contract.

This will include execution of a government furnished utility outage coordination form, which must be reviewed and signed by representatives of all appropriate base organization prior to approval by the contracting officer’s representative. The contractor will be provided a list of building managers and other appropriate points of contact, and will be given assistance by the government on an as needed basis. It shall be the contractor’s responsibility to initiate and coordinate utility outages forms. Coordination of the outage form must be completed a minimum of fourteen (14) calendar days prior to the requested outage date, and may require coordination with as many as twenty (20) individuals located on or near base. Any unusual delays in obtaining clearance from any particular organization shall be reported immediately to the contracting officer or contracting officer’s representative for assistance. Submit three (3) copies of the completed utility outage form to Civil Engineering construction management. Wide scale disruption of utility service to the military family housing areas shall include coordination with the base newspaper.

Following completion of the outage coordination, the contractor shall request shutdown from the utility company or Civil Engineer shop responsible for the service. The contractor shall pay any fees associated with the required shutdown and shall comply with all instructions pertaining to the shutdown. Once work begins on an approved outage, work must continue without interruption until the contractor restores utility services to the affected line(s) and/or facility.

24. DD FORM 1354, REAL ESTATE CHECKLIST: A DD Form 1354, showing items which must be completed, will be provided to the contractor. The checklist consists of line items describing the various components of the project including quantities, cost and dimensions which must be provided by the contractor. The checklist must be completed and submitted to Civil Engineering for approval. Final payment on the contract will not be approved until the DD Form 1354 has been satisfactorily completed by the contractor.

25. NOTIFICATION REQUIREMENTS (USED FOR HOUSING AREAS ONLY):

25.1 Following award of the contract for the project, the Air Force will notify residents of the upcoming work and the residents' responsibilities concerning same. For exterior work, the contractor will be responsible for providing each resident with written notification at least five (5) calendar days prior to (1) when the road in front of their unit will be closed to traffic and / or (2) when work on their yard will begin.

Further, the contractor shall provide each resident with a written notification at least 24 hours prior to shutting off utility service to their unit. The notice shall clearly state the dates of road closure, utility service interruption, or when yard work will begin. For interior work, the contractor will be responsible for

providing each resident with an initial written notification 14 calendar days in advance of the work and a follow-up notification five calendar days before work starts. In all cases, a brief description of the work, how long the particular item of work is expected to last, and points of contact for the Air Force and contractor shall also be stated (the Air Force point of contact will be provided at the pre-construction conference).

25.2 The notifications shall be hand delivered to each resident. Where contact with the resident is not made, the notice shall be left on the door at that unit. A copy of the proposed notification shall be submitted to the Contracting Officer for approval prior to distribution. In addition, a copy of the actual notification given to each resident, including the dates, shall be provided to the Contracting Officer within one working day of when it is presented to the resident.

26. REFUSE COLLECTION: In housing areas where the streets are closed to traffic for construction, the Contractor shall collect and transport the refuse from the curb on trash collection days. The refuse shall be placed in containers approved by the contract refuse collector and located where the refuse trucks can pick it up. Such containers shall be provided by the contractor with approval of the Contracting Officer.

Trash cans/containers are not to be removed from the individual residence locations.

27. MOBILE PHONE: The contractor’s site representative (superintendent or foreman), throughout the execution of the project, shall be accessible via a mobile phone. The number to the phone shall be local and provided to the Contracting Officer prior to the start of work.

28. STORAGE UNITS: The contractor shall request the Contracting Officer’s approval for the siting, size and color of storage units, CONEX boxes, office/storage trailers and dumpsters. The Contractor shall not bring any such items to the job site without the Contracting Officer’s approval of all particulars noted above. A nameplate shall be visually displayed on the exterior of all storage units, with the following information: Contractor’s Company, name of Contractor’s Point(s) of Contact (POC) and telephone number(s) where he/she can be reached, (during and after normal working hours) and Contract number.

29. SITE MAINTENANCE: The contractor shall keep the construction site cleaned daily of all construction trash, trash generated by employees, debris, and/or demolished building materials. Grass and weed growth in excavated materials that are stockpiled on site shall be controlled and / or cut weekly.

Weeds and grass within the construction site shall also be controlled and / or cut weekly. For the duration of the project and on a daily basis, Contractor shall remove from all base roads and / or streets mud, soil, rocks, trash and debris that result from his / her construction operation on base. All roads and / or streets affected shall be cleaned before close of business on the particular day affected.

29.1 Contractor or Contractor’s employees, including subcontractors, shall not park any personally owned vehicles (POVs) or company and / or delivery vehicles on grassy areas near or in the construction site without prior approval from the CO, or his / her designated representative. When allowed, parking on grassy areas will only be for short-term delivery purposes (to include heavy tools, equipment, construction materials, etc.). Use designated hard surfaces or existing parking lots near or within the project site for parking of POVs or company and / or delivery vehicles. Permission to park on grassy areas does not relieve the contractor of the responsibility to restore affected areas to original condition. Tire and track impressions (ruts) created on wet or soft soil by vehicles and / or equipment used in the Contractor’s operation shall be repaired. Contractor shall restore the affected area(s) to its original condition within a timely manner. Final payment shall not be made until all repairs have been completed to satisfaction of the Contracting Officer.

29.2 When areas of the site are raked prior to seeding (or sodding) or for leveling of topsoil to rid the site of mounds, clods and/or ruts, the Contractor shall ensure that no clods larger than 1” are left on the site.

Contractor shall restore the site to a condition and appearance similar or equal to existing before the damages occurred. Such restoration work will not be considered complete until approved by the CO or

his / her designated representative. The Government will not accept sites that have not been raked and restored to this standard.

29.3 At any given construction site, the Contractor shall grade, rake, and seed (or sod), or prepare for seeding (or sodding), the job site as soon as possible following work at that particular site. Grass areas disturbed by construction and/or those areas required to be “re-seeded” shall be re-established by drilling in seed then hydromulching only, unless otherwise specified.

SUBMITTAL PROCEDURES

PART 1 - GENERAL

1.1 RELATED DOCUMENTS

A. Drawings and general provisions of the Contract, including other Division 01

Specification Sections, apply to this Section.

1.2 SUMMARY

A. This Section includes administrative and procedural requirements for submittals required for performance of the Work, including the following:

1. Contractor's construction schedule.

2. Submittal schedule.

3. Daily construction reports.

4. Shop Drawings.

5. Product Data.

6. Samples.

7. Quality assurance submittals.

B. Related Sections: The following Sections contain requirements that relate to this Section:

1. Section 001000 "Ellsworth Air Force Base Special Provisions" specifies requirements for submittal of plans and permits for operating on Ellsworth AFB, SD.

2. Section 001400 "Quality Control" specifies requirements for submittal of inspection and test reports.

3. Section 001700 "Contract Closeout" specifies requirements for submittal of Project Record Documents and warranties at project closeout.

1.3 DEFINITIONS

A. Coordination Drawings show the relationship and integration of different construction elements that require careful coordination during fabrication or installation to fit in the space provided or to function as intended.

B. Field samples are full-size physical examples erected on-site to illustrate finishes, coatings, or finish materials. Field samples are used to establish the standard by which the Work will be judged.

C. Mockups are full-size assemblies for review of construction, coordination, testing, or operation; they are not Samples.

1.4 SUBMITTAL PROCEDURES

A. The Contractor shall fill in the REQUIRED SUBMISSION DATE column on the attached

Schedule of Material Submittals. Submit this document with AF Form 3064, Contract Progress Schedule, and provide both schedules to the Contracting Officer at the pre-

construction conference. Update the required submission date column as the project progresses. Prior to incorporating a particular item into the work, the submittal for that item must be approved.

B. Submittal Preparation: Place each submittal on an attached AF Form 3000 for identification. Submittals are to be provided in at least 4 copies, signed by the prime contractor. The Government will retain 2 copies and return the others.

C. Submittal Transmittal: Package each submittal appropriately for transmittal and handling. Transmit each submittal from the Contractor via the Contracting Officer to Civil Engineering Construction Management using an AF Form 3000. The Contracting Officer will not accept submittals received from sources other than the Contractor.

1. On the transmittal, record relevant information and requests for data. On the form, or separate sheet, record deviations from Contract Document requirements, including variations and limitations. Include Contractor's certification that information complies with Contract Document requirements.

D. Processing: To avoid the need to delay installation as a result of the time required to process submittals, allow sufficient time for submittal review, including time for resubmittals.

1. Allow 2 weeks for initial review. Allow additional time if the Civil Engineering

Construction Management must delay processing to permit coordination with subsequent submittals.

2. Allow 2 weeks for reprocessing each submittal.

3. No extension of Contract Time will be authorized because of failure to transmit submittals to Civil Engineering Construction Management sufficiently in advance of the Work to permit processing.

1.5 DAILY CONSTRUCTION REPORTS

A. Prepare a daily construction report recording the following information concerning events at the site, and submit duplicate copies to the Contracting Officer at weekly intervals:

1. List of subcontractors at the site.

2. Approximate count of personnel at the site.

3. High and low temperatures, general weather conditions.

4. Accidents and unusual events.

5. Meetings and significant decisions.

6. Stoppages, delays, shortages, and losses.

7. Emergency procedures.

8. Orders and requests of governing authorities.

9. Change Orders received, implemented.

10. Services connected, disconnected.

11. Equipment or system tests and startups.

12. Partial Completions, occupancies.

13. Substantial Completions authorized.

1.6 SHOP DRAWINGS

A. Submit newly prepared information drawn accurately to scale. Highlight, encircle, or otherwise indicate deviations from the Contract Documents. Do not reproduce Contract

Documents or copy standard information as the basis of Shop Drawings. Standard information prepared without specific reference to the Project is not a Shop Drawing.

B. Shop Drawings include fabrication and installation Drawings, setting diagrams, schedules, patterns, templates and similar Drawings. Include the following information:

1. Dimensions.

2. Identification of products and materials included by sheet and detail number.

3. Compliance with specified standards.

4. Notation of coordination requirements.

5. Notation of dimensions established by field measurement.

6. Sheet Size: Except for templates, patterns and similar full-size Drawings, submit

Shop Drawings on sheets at least 8-1/2 by 11 inches but no larger than 24 by 36 inches.

7. Do not use Shop Drawings without an appropriate final stamp indicating action taken and final approval by the Contracting Officer on AF Form 3000.

1.7 PRODUCT DATA

A. Collect Product Data into a single submittal for each element of construction or system.

Product Data includes printed information, such as manufacturer's installation instructions, catalog cuts, standard color charts, roughing-in diagrams and templates, standard wiring diagrams, and performance curves.

1. Mark each copy to show applicable choices and options. Mark copies to indicate the applicable information, where printed Product Data includes information on several products that are not required. Include the following information:

a. Manufacturer's printed recommendations.

b. Compliance with trade association standards.

c. Compliance with recognized testing agency standards.

d. Application of testing agency labels and seals.

e. Notation of dimensions verified by field measurement.

f. Notation of coordination requirements.

2. Do not submit Product Data until compliance with requirements of the Contract

Documents has been confirmed.

1.8 SAMPLES

A. Submit full-size, fully fabricated Samples cured and finished as specified and physically identical with the material or product proposed. Samples include partial sections of manufactured or fabricated components, cuts or containers of materials, color range sets, and swatches showing color, texture, and pattern.

1. Mount or display Samples in the manner to facilitate review of qualities indicated.

a. Specification Section number and reference.

b. Generic description of the Sample.

c. Sample source.

d. Product name or name of the manufacturer.

e. Compliance with recognized standards.

f. Availability and delivery time.

2. Submit Samples for review of size, kind, color, pattern, and texture. Submit

Samples for a final check of these characteristics with other elements and a

comparison of these characteristics between the final submittal and the actual component as delivered and installed.

a. Where variation in color, pattern, texture, or other characteristic is inherent in the material or product represented, submit at least 3 multiple units that show approximate limits of the variations.

b. Refer to other Specification Sections for requirements for Samples that illustrate workmanship, fabrication techniques, details of assembly, connections, operation, and similar construction characteristics.

c. Refer to other Sections for Samples to be returned to the Contractor for incorporation in the Work. Such Samples must be undamaged at time of use. On the transmittal, indicate special requests regarding disposition of Sample submittals.

d. Samples not incorporated into the Work, or otherwise designated as the Government's property, are the property of the Contractor and shall be removed from the site prior to Substantial Completion.

3. Maintain sets of Samples, as returned, at the Project Site, for quality comparisons throughout the course of construction.

a. Sample sets may be used to obtain final acceptance of the construction associated with each set.

B. Distribution of Samples: Prepare and distribute additional sets to subcontractors, manufacturers, fabricators, suppliers, installers, and others as required for performance of the Work.

1. Field samples are full-size examples erected on-site to illustrate finishes, coatings, or finish materials and to establish the Project standard.

a. Comply with submittal requirements to the fullest extent possible. Process transmittal forms to provide a record of activity.

1.9 QUALITY ASSURANCE SUBMITTALS

A. Submit current year, quality-control submittals, including design data, certifications, manufacturer's instructions, manufacturer's field reports, and other quality-control submittals as required under other Sections of the Specifications.

B. Certifications: Where other Sections of the Specifications require certification that a product, material, or installation complies with specified requirements, submit a current year, notarized certification from the manufacturer certifying compliance with specified requirements.

1. Signature: Certification shall be signed by an officer of the manufacturer or other individual authorized to sign documents on behalf of the company.

C. Inspection and Test Reports: Requirements for submittal of inspection and test reports from independent testing agencies are specified in Section 001400 "Quality Control."

1.10 CONTRACTING OFFICER ACTION

A. Except for submittals for the record or information, where action and return is required, the Contracting Officer will review each submittal, mark the AF Form 3000 to indicate action taken, and return promptly.

1. Compliance with specified characteristics is the Contractor's responsibility.

2. Resubmittal: When the Contracting Officer marks a submittal "Disapproved", do not proceed with Work covered by the submittal, including purchasing, fabrication, delivery, or other activity. Revise or prepare a new submittal according to the notations; resubmit without delay. Repeat if necessary to obtain different action mark.

a. Do not use, or allow others to use, submittals marked "Disapproved" at the

Project Site or elsewhere where Work is in progress.

B. Unsolicited Submittals: The Contracting Officer will return unsolicited submittals to the sender without action.

INSTRUCTIONS TO CONTRACTORS

1. The term "material" is defined as articles, supplies, raw materials, equipment, parts, components, and end items that are to be incorporated into the work required by the contract.

2. This form is to be used by contractors for submitting Shop Drawings, Equipment Data, Manufacturer's Literature and Certificates and samples of Materials to the Government for approval in accordance with the provisions of this contract. Unless otherwise specified, it is to be prepared in 4 copies, signed, and provided to the contracting officer with appropriate attachments.

3. Item(s) to be approved will be clearly tabbed or identified. Data pertaining to item(s) to be approved will be clearly identified or tabbed, particularly where documents are voluminous, in order to properly evaluate the materials or articles to be incorporated in the work. Each attachment will be numbered to correspond with the item number shown on the face of this form.

4. Requests submitted shall be numbered consecutively, by contract, in the space entitled "Submission No.". This number, in addition to the Contract No., will be used to identify each Material Approval Submittal. Resubmissions will be indicated in the appropriate block and the insertion of previous submission number and data in addition to a new submission number. A single submission should be used for all work of a section of the specifications, but in NO instance should the submission include work for more than one (1) contract. Submittals requiring priority handling will be submitted by separate submittal using the form and so marked across the face of the form.

CONTRACT CLOSEOUT

1.1 RELATED DOCUMENTS

A. Drawings and general provisions of the Contract, including other Division 01

Specification Sections, apply to this Section.

1.2 SUMMARY

A. This Section includes administrative and procedural requirements for contract closeout including, but not limited to, the following:

1. Inspection procedures.

2. Project record document submittal.

3. Operation and maintenance manual submittal.

4. Submittal of warranties.

5. Final cleaning.

B. Closeout requirements for specific construction activities are included in the appropriate

Sections in Divisions 02 through 33.

1.3 SUBSTANTIAL COMPLETION

A. Preliminary Procedures: Before requesting inspection for certification of Substantial

Completion, complete the following. List exceptions in the request.

1. Submit specific warranties, maintenance agreements, final certifications, and similar documents.

2. Submit record drawings, maintenance manuals, final project photographs, damage or settlement surveys, property surveys, and similar final record information.

3. Deliver tools, spare parts, extra stock, and similar items.

4. Make final changeover of permanent locks and transmit keys to the Contracting

Officer. Advise Government personnel of changeover in security provisions.

5. Complete startup testing of systems and instruction of Government operation and maintenance personnel. Discontinue and remove temporary facilities from the site, along with mockups, construction tools, and similar elements.

6. Complete final cleanup requirements, including touchup painting.

7. Touch up and otherwise repair and restore marred, exposed finishes.

B. Inspection Procedures: On receipt of a request for inspection, the Contracting Officer will either proceed with inspection or advise the Contractor of unfilled requirements.

1. The Contracting Officer will repeat inspection when requested and assured that the Work is substantially complete.

2. Results of the completed inspection will form the basis of requirements for final acceptance.

1.4 FINAL ACCEPTANCE

A. Preliminary Procedures: Before requesting final inspection Contractor shall submit a copy of previous inspection list showing completed action.

B. Reinspection Procedure: The Contracting Officer will reinspect the Work upon receipt of notice that the Work, including inspection list items from earlier inspections, has been completed, except for items whose completion is delayed under circumstances acceptable to the Contracting Officer.

1. If the Work is incomplete, the Contracting Officer will advise the Contractor of

Work that is incomplete or of obligations that have not been fulfilled but are required for final acceptance.

2. If necessary, reinspection will be repeated.

1.5 RECORD DOCUMENT SUBMITTALS

A. General: Do not use record documents for construction purposes. Protect record documents from deterioration and loss in a secure, fire-resistant location. Provide access to record documents for the Contracting Officer's reference during normal working hours.

B. Record Drawings: Refer to section 001050 within these specifications.

C. Miscellaneous Record Submittals: Refer to other Specification Sections for requirements of miscellaneous record keeping and submittals in connection with actual performance of the Work. Immediately prior to the date or dates of Substantial Completion, complete miscellaneous records and place in good order. Identify miscellaneous records properly and bind or file, ready for continued use and reference.

Submit to the Contracting Officer for the Government's records.

D. Maintenance Manuals: Organize operation and maintenance data into suitable sets of manageable size. Bind properly indexed data in individual, heavy-duty, 2-inch (51-mm), 3-ring, vinyl-covered binders, with pocket folders for folded sheet information. Mark appropriate identification on front and spine of each binder. Include the following types of information:

1. Emergency instructions.

2. Spare parts list.

3. Copies of warranties (address and telephone number of authorized representative).

4. Wiring diagrams.

5. Recommended "turn-around" cycles.

6. Inspection and maintenance procedures.

7. Shop Drawings and Product Data.

8. Fixture lamping schedule.

9. Operating instructions.

E. A DD Form 1354 Checklist will be provided to the contractor with the required areas highlighted that must be completed. The Contractor is required to submit the completed checklist at the time of the final inspection. The checklist consists of line items describing the various components of the project including quantities, cost and dimensions which must be provided by the Contractor. The checklist must be completed to the satisfaction of the Contracting Officer prior to the final payment.

Operation and Maintenance Instructions: Arrange for each Installer of equipment that requires regular maintenance to meet with Government personnel to provide instruction in proper operation and maintenance. Provide instruction by manufacturer's representatives if installers are not experienced in operation and maintenance procedures. Instructions shall be scheduled for a Tuesday, Wednesday or Thursday.

Contractor shall arrange the day and time of the instruction with the project inspector.

Include a detailed review of the following items:

1. Maintenance manuals.

2. Record documents.

3. Spare parts and materials.

4. Tools.

5. Lubricants.

6. Fuels.

7. Identification systems.

8. Control sequences.

9. Hazards.

10. Cleaning.

11. Warranties.

12. Maintenance agreements and similar continuing commitments.

B. As part of instruction for operating equipment, demonstrate the following procedures:

1. Startup.

2. Shutdown.

3. Emergency operations.

4. Noise and vibration adjustments.

5. Safety procedures.

6. Economy and efficiency adjustments.

7. Effective energy utilization.

3.2 FINAL CLEANING

A. General: The General Conditions require general site cleaning during construction on a daily basis.

B. Cleaning: Employ experienced workers or professional cleaners for final cleaning.

Clean each surface or unit to the condition expected in a normal, commercial building cleaning and maintenance program. Comply with manufacturer's instructions.

1. Complete the following cleaning operations before requesting final inspection.

a. Remove labels that are not permanent labels.

b. Clean transparent materials, including mirrors and glass in doors and windows. Remove glazing compounds and other substances that are noticeable vision-obscuring materials. Replace chipped or broken glass and other damaged transparent materials.

c. Clean exposed exterior and interior hard-surfaced finishes to a dust-free condition, free of stains, films, and similar foreign substances. Restore

reflective surfaces to their original condition. Leave concrete floors broom clean. Vacuum carpeted surfaces.

d. Wipe surfaces of mechanical and electrical equipment. Remove excess lubrication and other substances. Clean plumbing fixtures to a sanitary condition. Clean light fixtures and lamps.

e. Clean the site, including landscape development areas, of rubbish, litter, and other foreign substances. Sweep paved areas broom clean; remove stains, spills, and other foreign deposits. Rake grounds that are neither paved nor planted to a smooth, even-textured surface.

C. Pest Control: Engage an experienced, licensed exterminator to make a final inspection and rid the Project of rodents, insects, and other pests.
