1.
PRINCIPAL FEATURES: The work covered by these specifications and accompanying drawings consists of furnishing all plant, labor, materials, tools, and equipment necessary in performing all operations in connection with Project Tririga # XXXXXXX(Project number), (Insert project title here).
2.
SCOPE: In general, the principal features of the work are outlined but not limited to this paragraph. Unless otherwise indicated, it is not the intent of this listing to include technical data for materials or the installation of these materials. The project includes (insert the description of the project here).
3.
FIELD MEASUREMENTS: The Contractor shall verify all measurements at the site and shall be responsible for dimensions and proper installation of all materials.
4. AVAILABILITY OF UTILITIES: All reasonable quantities of water, natural gas, and electricity will be made available to the contractor without charge. Connections to the utilities are the contractor’s responsibility and shall be coordinated with, and approved by, the appropriate Civil Engineering utility shop. The water fill stand located north of facility 7510 may be used for filling tank trucks. If connections are made to base fire hydrants, the contractor shall notify the base fire department and install a backflow prevention device as part of the connection. The Contractor shall carefully conserve any utilities furnished without charge. Before final acceptance of the work by the Government, the Contractor shall remove all the temporary connections, distribution lines, and associated paraphernalia.
5.
CONTINUED USE OF FACILITIES: All contractor operations must be closely coordinated with the using agency. The Contractor shall not start work before 6:30 a.m. each morning. All utilities shall remain in operation during construction unless specifically noted otherwise.
6.
SEQUENCE OF WORK: Work under this contract shall be sequenced and performed in accordance with the following requirements. (Contractor to insert any relevant sequencing information in this section.)
7.
DISPOSAL OF MATERIALS: Unless otherwise directed, the contractor shall remove all excess materials, except contaminated soil, from the base in such a manner that meets federal, state and local environmental codes. Contaminated soil will be deposited at a location designated by the government. At the conclusion of each project, the contractor will be responsible for grading the stockpile area for proper drainage and restoring it by seeding and establishing grass cover. Loose debris on trucks leaving project sites shall be loaded in a manner that will prevent spillage from the vehicle. Fasten suitable cover, such as a tarpaulin, over the load before entering surrounding streets. The contractor shall be responsible for immediately cleaning up any materials that fall from trucks and held liable for any resulting damages. Contractor shall submit, upon request, all trip tickets from the landfill facility to show all debris is being disposed of in accordance with all Federal requirements and in an approved location.
8.
CONTRACTOR SUBMITTALS: Submittals are to be signed by the contractor and provided in accordance with the requirements of other portions of the contract and the Submittal Schedule.
9. CONSTRUCTION PERMITS
9.1 Digging Permits: The contractor will be responsible for coordinating a government supplied, Base Civil Engineer Work Clearance Request (AF Form 103) prior to performing digging of any type. The contractor shall process the digging permit by coordinating with and obtaining signatures from responsible representatives of the organizations listed on the AF Form 103 prior to obtaining final approval from the Air Force Base Civil Engineer or his approved representative. The contractor will be given assistance, by the government, in the execution of the initial Work Clearance Request. Thereafter, government assistance will be limited to an as-needed basis in the event of unusual circumstances. It will be the contractor's responsibility to coordinate completion of the necessary AF Form 103 and arrange to have existing utilities located as indicated on the completed form, prior to the beginning of digging operations in the individual areas. This coordination is anticipated to take approximately five (5) working days to complete per request, and may require coordination with as many as twenty (20) individuals located on or near the base. A copy of the AF Form 103 is attached for reference. Any unusual delay in obtaining approval from any particular organization will be reported immediately to the contracting officer’s representative for assistance.
9.2 Utility Staking Requirements: The contractor shall layout and mark his intended area(s) of excavation before calling for field coordination by utility personnel. This shall be done a minimum of five working days in advance of when digging is expected to begin. Once all responsible utility representatives have field located crossover and/or interference points between the proposed work and existing utilities, and signed off on the digging permit to signify completion of the field coordination of the digging permit, then digging in the area represented by the digging permit may begin. Any utility service markers or markings established by the utility representatives must be maintained by the contractor through the completion of the digging operations. As soon as work requiring the markers is completed, all above-ground marking devices (flags / stakes / etc) shall be removed by the contractor.
9.3 Digging Operations: Digging near established interference or crossover points shall be done by hand, five (5) feet either side of the point along the intended route, in order to prevent disturbing the existing utility. If the existing utility is uncovered in the new excavation, it shall be protected from damage and movement while in the open excavation and during backfill. The contractor shall be responsible for the repairs and associated costs for repairs of any utility damaged by construction, whose location was made known to the contractor.
10.
LAYOUT OF PROJECT: All surveying and layout work is the responsibility of the contractor. The government will assist the contractor only in providing elevations or location to begin the work.
11.
SAMPLING AND TESTING: All quality control sampling and testing shall be the responsibility of the contractor. Sampling and testing shall be performed by an approved commercial testing laboratory. Copies of all test results shall be submitted to the contracting officer.
12.
IDENTIFICATION OF EMPLOYEES
(a) Entry Authority List (EAL): These lists are kept at the gates to verify employment. The following guidelines apply:
(1) On Base: Original EAL and two (2) copies to the Contracting Office is required
(2) The EAL must be in alphabetical order, on company letterhead, containing the complete address and phone number of the company, signed by an authorized company representative, and in the following format:

(3) DO NOT combine your EAL with the subcontractor's EAL. Each company/subcontractor must have a separate EAL.
(4) Additions/Deletions of Personnel: When personnel are to be added or deleted, the entire EAL shall be re-accomplished and submitted to the Contracting Officer.
(5) Escorts: An asterisk (*) on the EAL by key personnel such as a foreman or supervisor will allow temporary entry of supply deliveries / pickup / repair. The escorts must accompany these personnel the duration of time on the installation. A maximum of three (3) key personnel per contractor is allowed. A phone number where the escorts can be reached must be included on the EAL.
(6) Two (2) days are required for processing of EALs.
(b) Air Force Form 75, Visitors Pass (AF 75): The AF 75 is required for all employees and vehicles, The AF 75 has two parts, one for placement on the driver's side of the dashboard (must be visible at all times), the other part is issued to the individual and must be on his/her person at all times. Each employee must obtain this pass by presenting his drivers license at the Visitor Control Center, located outside the Liberty Gate. If any of the above identification is not on the employee’s person, he/she will be escorted from the base. It is recommended that a magnetic sign be placed on the left door of the vehicles that do not have a company logo. Periodic spot checks of drivers licenses and AF 75's will be conducted on a random basis.
13 CONTRACTORS STORAGE AREA: The contractor will be authorized the use of an area adequate for his materials and equipment storage at the site shown on the Title Sheet of the drawings. The contractor shall set up and maintain his operation within the area in an orderly manner and upon completion of the project, shall restore the premises to its original condition free of all excess materials, debris and excavations or embankments resulting from his operations. The contractor will also be responsible for his own equipment security.
14. TEMPORARY STAGING AREA: The contractor may be allowed to utilize certain locations within the project area as approved by the contracting officer to park equipment and store construction materials. Staging areas shall be maintained in a neat, orderly manner including erection of temporary fencing around the areas and restored after use.
15.
CONTRACTOR'S HAUL ROUTES: Access to the project will be by Bismark (Commercial) Gate during the week (Monday through Friday), and Liberty (Main) Gate during weekends (Saturday and Sunday), thence over the haul route indicated on the Location Plan of the drawings. Work during weekends and holidays will only be allowed with the approval of the contracting officer or his designated representative. The designated routes shall be used by both loaded and unloaded vehicles.
16.
TEMPORARY TOILETS: Unless otherwise directed by the contract documents, the contractor shall provide and maintain sanitary temporary toilets for all work accomplished under the project. These toilets shall be located as directed by the contracting officer. The temporary toilets shall be portable, enclosed and weatherproofed and kept in a sanitary condition at all times.
17.
CONSTRUCTION SIGNAGE: The contractor shall furnish, install and maintain traffic control signage for street and highway construction operations per the requirements of the Manual on Uniform Traffic Control Devices for Street and Highways. Prior to the contractor beginning construction, a detailed traffic control plan showing all construction signage shall be submitted to the contracting officer for review and comment.
18.
EXISTING CONDITIONS: The plans represent the best available information at the time they were prepared. While every effort has been made to correctly show all major existing items which will affect the work, it is recommended that prospective bidders examine the site prior to submitting a bid. No additional payment will be made for extra work required by obstacles which were in place and clearly visible at the time of bidding.
19.
WORK SCHEDULE. Working hours for the contractor will normally be between the hours of 6:30 a.m. and 5:00 p.m. excluding Saturdays, Sundays and federal holidays. If the contractor desires to work during periods other than above, additional government inspection forces may be required. The contractor must make his / her request to the contracting officer three days in advance of his / her intention to work during other periods to allow assignment of additional inspection personnel. The contracting officer shall determine, at his discretion, whether such inspection forces are available and whether work will be allowed on a non-scheduled day. The contracting officer may authorize the contractor to perform work during periods other than normal duty hours / days; however, if inspectors are required to perform in excess of their normal duty hours / days solely for the benefit of the contractor, the actual cost of inspection at overtime rates will be charged to the contractor. These adjustments to the contract price may be made periodically as directed by the contracting officer.
20.
DAILY CLEAN-UP: The contractor shall at all time keep the premises free from accumulations of waste materials or rubbish caused by his employees or work. At completion of each work day, he shall remove all rubbish from the construction site. All his tools, equipment, and surplus materials, etc., shall be stored in the storage area or temporary staging area.
21. CONTRACTOR DAMAGES: Areas not identified to be removed / worked under this contract that are disturbed or damaged during construction shall be removed and replaced at no additional cost to the Government.
22.
UTILITY ADJUSTMENTS: The contractor shall adjust or shall arrange to have adjusted, utilities that are in conflict with new construction. The contractor shall be responsible for adjusting any water, gas, communication line, and electrical lines. The contractor shall employ qualified personnel to perform these adjustments.
23. UTILITY OUTAGES: The contractor shall perform the work under this contract with a minimum of outage time for all utilities. If possible, the contractor shall schedule any work that requires utility outages (electrical, water, gas, etc.) which will close down or limit normal activities in the building, construction area, or other affected areas at a time other than regular work periods of the organization occupying the facility. The contractor shall be responsible for coordination of all utility outages executed for this contract. This will include execution of a government furnished utility outage coordination form, which must be reviewed and signed by representatives of all appropriate base organization prior to approval by the contracting officer’s representative. The contractor will be provided a list of building managers and other appropriate points of contact, and will be given assistance by the government on an as needed basis. It shall be the contractor’s responsibility to initiate and coordinate utility outages forms. Coordination of the outage form must be completed a minimum of fourteen (14) calendar days prior to the requested outage date, and may require coordination with as many as twenty (20) individuals located on or near base. Any unusual delays in obtaining clearance from any particular organization shall be reported immediately to the contracting officer or contracting officer’s representative for assistance. Submit three (3) copies of the completed utility outage form to Civil Engineering construction management. Wide scale disruption of utility service to the military family housing areas shall include coordination with the base newspaper. Following completion of the outage coordination, the contractor shall request shutdown from the utility company or Civil Engineer shop responsible for the service. The contractor shall pay any fees associated with the required shutdown and shall comply with all instructions pertaining to the shutdown. Once work begins on an approved outage, work must continue without interruption until the contractor restores utility services to the affected line(s) and/or facility.
24. DD FORM 1354, REAL ESTATE CHECKLIST: A DD Form 1354, showing items which must be completed, will be provided to the contractor. The checklist consists of line items describing the various components of the project including quantities, cost and dimensions which must be provided by the contractor. The checklist must be completed and submitted to Civil Engineering for approval. Final payment on the contract will not be approved until the DD Form 1354 has been satisfactorily completed by the contractor.
25. NOTIFICATION REQUIREMENTS (USED FOR HOUSING AREAS ONLY):
25.1 Following award of the contract for the project, the Air Force will notify residents of the upcoming work and the residents' responsibilities concerning same. For exterior work, the contractor will be responsible for providing each resident with written notification at least five (5) calendar days prior to (1) when the road in front of their unit will be closed to traffic and / or (2) when work on their yard will begin. Further, the contractor shall provide each resident with a written notification at least (24) hours prior to shutting off utility service to their unit. The notice shall clearly state the dates of road closure, utility service interruption, or when yard work will begin. For interior work, the contractor will be responsible for providing each resident with an initial written notification 14 calendar days in advance of the work and a follow-up notification five calendar days before work starts. In all cases, a brief description of the work, how long the particular item of work is expected to last, and points of contact for the Air Force and contractor shall also be stated (the Air Force point of contact will be provided at the pre-construction conference).
25.2 The notifications shall be hand delivered to each resident. Where contact with the resident is not made, the notice shall be left on the door at that unit. A copy of the proposed notification shall be submitted to the Contracting Officer for approval prior to distribution. In addition, a copy of the actual notification given to each resident, including the dates, shall be provided to the Contracting Officer within one working day of when it is presented to the resident.
26. REFUSE COLLECTION: In housing areas where the streets are closed to traffic for construction, the Contractor shall collect and transport the refuse from the curb on trash collection days. The refuse shall be placed in containers approved by the contract refuse collector and located where the refuse trucks can pick it up. Such containers shall be provided by the contractor with approval of the Contracting Officer. Trash cans/containers are not to be removed from the individual residence locations.
27. MOBILE PHONE: The contractor’s site representative (superintendent or foreman), throughout the execution of the project, shall be accessible via a mobile phone. The number to the phone shall be local and provided to the Contracting Officer prior to the start of work.
28. STORAGE UNITS: The contractor shall request the Contracting Officer’s approval for the siting, size and color of storage units, CONEX boxes, office/storage trailers and dumpsters. The Contractor shall not bring any such items to the job site without the Contracting Officer’s approval of all particulars noted above. A nameplate shall be visually displayed on the exterior of all storage units, with the following information: Contractor’s Company, name of Contractor’s Point(s) of Contact (POC) and telephone number(s) where he/she can be reached, (during and after normal working hours) and Contract number.
29. SITE MAINTENANCE: The contractor shall keep the construction site cleaned daily of all construction trash, trash generated by employees, debris, and/or demolished building materials. Grass and weed growth in excavated materials that are stockpiled on site shall be controlled and / or cut weekly. Weeds and grass within the construction site shall also be controlled and / or cut weekly. For the duration of each DO and on a daily basis, Contractor shall remove from all base roads and / or streets mud, soil, rocks, trash and debris that result from his / her construction operation on base. All roads and / or streets affected shall be cleaned before close of business on the particular day affected.
Contractor or Contractor’s employees, including subcontractors, shall not park any personally owned vehicles (POVs) or company and / or delivery vehicles on grassy areas near or in the construction site without prior approval from the CO, or his / her designated representative. When allowed, parking on grassy areas will only be for short-term delivery purposes (to include heavy tools, equipment, construction materials, etc.). Use designated hard surfaces or existing parking lots near or within the project site for parking of POVs or company and / or delivery vehicles. Permission to park on grassy areas does not relieve the contractor of the responsibility to restore affected areas to original condition. Tire and track impressions (ruts) created on wet or soft soil by vehicles and / or equipment used in the Contractor’s operation shall be repaired. Contractor shall restore the affected area(s) to its original condition within a timely manner. Final payment shall not be made until all repairs have been completed to satisfaction of the Contracting Officer.
When areas of the site are raked prior to seeding (or sodding) or for leveling of topsoil to rid the site of mounds, clods and/or ruts, the Contractor shall ensure that no clods larger than 1” are left on the site. Contractor shall restore the site to a condition and appearance similar or equal to existing before the damages occurred. Such restoration work will not be considered complete until approved by the CO or his / her designated representative. The Government will not accept sites that have not been raked and restored to this standard.
At any given construction site, the Contractor shall grade, rake, and seed (or sod), or prepare for seeding (or sodding), the job site as soon as possible following work at that particular site. Grass areas disturbed by construction and/or those areas required to be “re-seeded” shall be re-established by drilling in seed then hydromulching only, unless otherwise specified.
SECTION 001800 - ENVIRONMENTAL PROVISIONS
PART 1 - GENERAL

1.2
SUMMARY
A.
This section contains administrative and procedural requirements for environmental issues.
1.3
DEFINITIONS
Operable units: Operable Units (OU) are defined as a discrete response action within a comprehensive environmental restoration program. OUs are areas that have been shown by conceptual models of contaminant sources, migrations, and receptors to require specific source control and/groundwater response actions. Maps showing the locations of the OUs on EAFB can be obtained from the AFCEC/CZOM office at 2125 Scott Drive, (605) 385-2688.

1.5
STORM WATER POLLUTION PREVENTION REQUIREMENTS
A.
Permits:
1. Storm Water Construction Permit Coverage Required. If the construction activities include clearing, grading, stockpiling or excavation activities which result in the disturbance of one or more acres of total land area, the project will require a South Dakota Department of Environment and Natural Resources (DENR) General Permit for Storm Water Discharges Associated with Construction Activities. The landowner gets the permit and the contractor becomes a co-permittee.
a. Permit Acquisition. The permit acquisition process begins with contractor preparation and submittal of a Storm Water Pollution Prevention Plan (SWPPP); The SWPPP preparer uses the EPA template loaded on the DENR stormwater site (at http://denr.sd.gov/des/sw/SWPPPRequirements.aspx). The SWPPP preparer will ensure the plan meets all requirements by performing a checklist inspection of the plan using the “South Dakota NPDES Storm Water Inspection – Construction” checklist. The preparer will correct all deficiencies. The SWPPP shall be revised by the site operator as construction proceeds
b. The contractor will submit the developed SWPPP, the checklist used to ensure completeness, and a signed SD Contractor Certification Form (original, not copy). Once the Air Force receives and approves these submittals, the base will submit the Notice of Intent to DENR for the activity to be covered under the South Dakota General Permit for Storm Water Discharges Associated with Construction Activities. No field work can be started until the base gets the letter from DENR stating they are covered under the permit. Failure by the contractor to comply with the above subject permit will result in work stoppage at no cost to the government.
2.
Site Dewatering. If the project will involve dewatering which will reach the waters of the state, the contractor shall apply to be covered under the SD DENR General Permit to Discharge under the Surface Water Discharge System for Temporary Dewatering Activities in South Dakota. The contractor will be responsible to acquire and maintain compliance with the permit.
3.
Records. The site operator(s) will ensure that copies of all records (SWPPP updates, inspections, etc) shall be submitted to the contracting officer.
B.
Licensing/Certifications:
1. The SWPPP preparer must demonstrate training and certification to develop SW plans (SD DoT SWPPP design course or equivalent).
2. The construction contractor site operator must demonstrate training and certification to implement, maintain and inspect SWPPP plans (SD DoT Sed and Erosion Course or equivalent).
C.
Procedures: When the construction activities include clearing, grading, stockpiling or excavation activities which result in the disturbance of land cover of less than one acre the contractor shall develop and submit an abridged storm water pollution prevention plan using chapters 2 and 3 of the EPA template loaded on the DENR stormwater site (can be found at http://denr.sd.gov/des/sw/SWPPPRequirements.aspx). The SWPPP preparer will ensure the plan meets all requirements by performing a checklist inspection of the plan using the “South Dakota NPDES Storm Water Inspection – Construction” checklist. This plan shall be completed and implemented prior to the time the project breaks ground and revised (if necessary) as construction proceeds.
1.
Stabilization will be initiated on all disturbed areas where construction activity will not occur for a period of more than 21 calendar days. The stabilization work will begin by the 14th day after construction activity has permanently or temporarily ceased.
2.
Construction Related Excavation De-watering and Hydrostatic Tests. The contractor shall follow one of the following procedures for excavation de-watering and hydrostatic testing.
a.
The contractor can apply to be covered under the General Permit to Discharge under the National Pollutant Discharge Elimination System for Construction Activities under the current South Dakota permit (The EAFB Environmental Specifications Handbook contains this general permit). (If the permit is requested for an area in an operable unit the water quality data must be forwarded to the SD DENR along with the notice of intent.)
b.
The contractor can discharge the water to a level area at such a rate that it infiltrates and does not run off into a road ditch, storm drain or any drainage ditch. (This cannot be used for construction activities in operable units.)
c.
The water can be containerized and disposed of in the Sanitary Sewer System. (For construction activities in operable units the water must first be analyzed for the contaminates of concern for the specific Operable Unit with results reviewed and disposal approved by 28 CES/CEIEC prior to discharge).
3.
Road and Parking Lot Surfacing: The contractor shall be responsible for any and all releases from the construction site during road surfacing work such as, but not limited to, asphalt placement, chip sealing and concrete work. The contractor shall pay close attention to the weather forecast to ensure the effects of rain are minimized. The contractor shall have the personnel, material, and equipment available to ensure there is no release off the site into the storm drainage areas. If a release does occur, the contractor will be responsible for the clean-up.
4.
Stormwater Pollution Prevention Best Management Practices (BMPs): In accordance with the state-issued discharge permit, the installation is required to implement BMPs to minimize contamination of storm water by industrial activities. Current BMPs are identified in the EAFB SWPP Plan; however, activities not covered may require meeting with environmental personnel to establish an efficient practice.

C.
Procedures: The contractor shall report any spill of oil or hazardous material in accordance with the EAFB Spill Prevention Control and Countermeasure Plan. The contractor shall take every reasonable precaution to prevent the spillage of oil or other hazardous substances. Should a spill occur, the contractor shall be totally responsible for having the material and resources on hand for the containment and clean-up of the spilled material. The contractor shall perform all sampling to verify clean-up of the spill site and ensure proper disposal of the contaminated material, as required by the Contracting Officer or the South Dakota Department of Environment and Natural Resources (DENR). The contractor shall develop and have on site Spill Prevention, Control and Countermeasure Plan (SPCCP). The contractor shall provide a copy of the SPCCP to the Contracting Officer for review and approval prior to starting work.

The contractor shall analyze all wastes suspected to be hazardous wastes as defined by the Resource Conservation and Recovery Act (RCRA). Wastes determined to be RCRA hazardous wastes will be delivered to the Hazmart at Twining Street with a copy of the waste analysis in a Department of Transportation (DOT) approved container for proper handling and disposal.

The contractor shall analyze all oils and equipment suspected to be contaminated with Polychlorinated Biphenyls (PCBs). Materials contaminated with PCBs shall be turned over to the Environmental Element, 28 CES/CEIEC, for proper handling and disposal, telephone 385-2674.

Procedures:
1.
Prior to commencing work, the contractor shall review all possible waste streams with the Ellsworth AFB Hazardous Waste Program Manager. The Ellsworth AFB Hazardous Waste Program Manager will determine sampling requirements for all necessary hazardous constituents that may be present in the waste. The contractor shall properly sample all wastes, in accordance with EPA SW-846, suspected to be hazardous wastes as defined by the Resource Conservation and Recovery Act (RCRA). The contractor will provide proper containers to containerize all wastes for disposal. The contractor will provide containers suitable to meet Department of Transportation (DOT) shipping requirements for any hazardous waste generated during the project. Prior to use of the containers for collection of hazardous waste, the Ellsworth AFB Hazardous Waste Program Manager must approve the containers for this use. The contractor shall properly containerize any hazardous waste and deliver the containers with the RCRA hazardous wastes to the HAZMART located on Twining Street. The contractor will provide the HAZMART contractor with copies of all waste analysis reports, Safety Data Sheets (SDSs), and any other documentation used to characterize the waste.
2. Suspected presence of toxic materials shall be handled as follows:
a. The contractor will sample all oils and equipment suspected to be contaminated with Polychlorinated Biphenyls (PCBs). Materials contaminated with PCBs shall be turned over to the Hazardous Waste Contractor for proper disposal, telephone 385-6602. Any questions can be directed to the Environmental Element, 28 CES/CEIEC, telephone 385-6616.
b. The contractor will have all paints suspected to be Lead Based sampled by a certified individual. If results come back positive as Lead Based Paint, the abatement must be performed by a South Dakota certified abatement contractor IAW all Federal, State and Local Regulations. Materials contaminated with Lead shall be turned over to the Hazardous Waste Contractor for proper disposal, telephone 385-6602. Any questions can be directed to the Environmental Element, 28 CES/CEIEC, telephone 385-6616.
c. The contractor will cease all operations if material is encountered that is suspected to contain asbestos. The contractor will have a certified individual sample all suspect materials to see if it is Asbestos Containing Material (ACM). If it is ACM, abatement must be performed by a South Dakota certified abatement contractor IAW all Federal, State and Local Regulations. All ACM will be manifested and properly disposed of by the South Dakota Certified contractor and manifest will be turned over to the Environmental Element, 28 CES/CEIEC, telephone 385-6616. DISPOSAL OF MATERIALS
A.
Prior to the removal of waste materials from Ellsworth AFB under this contract, the contractor shall have all waste streams, recyclable materials, selected waste disposal facilities, or selected recycling facilities reviewed and approved by the Hazardous Waste Program Manager, Solid Waste Program Manager, and the Qualified Recycling Program (QRP) Manager. The contractor shall dispose of waste materials off base in such a manner that meets all federal, state and local environmental laws and regulations. (NO SOIL CAN BE REMOVED OFF SITE WITHOUT FIRST CONTACTING AFCEC/CZOM, THE ENVIRONMENTAL RESTORATION OFFICE, 385-2688).
1.9
INDUSTRIAL WASTE MANAGEMENT PROGRAM
A.
The contractor shall fill out and submit for approval a Waste Inventory Sheet prior to discharging any wastestream down an industrial or sanitary sewer drain.
1.10
CULTURAL RESOURCE PRESERVATION
A.
ARCHEOLOGICAL PRESERVATION: Should the Contractor uncover human remains or other skeletons, Native American artifacts or other archaeological resources during the course of excavation, the work shall be stopped and the Base Cultural Resources Manager in the 28 CES/CEIEC office at 2125 Scott Drive, (605) 385-2690 and the Contracting Officer shall be notified immediately.
B.
HISTORIC BUILDING PRESERVATION: If construction activities include excavation or construction which affect any identified historic building or structure, the contractor shall consult with the Base Cultural Resources Manager. A list of identified historic buildings is maintained and available through the 28 CES/CEIEC office at 2125 Scott Drive, (605) 385-2690.

NATURAL RESOURCE PRESERVATION AND PROTECTION
A.
WETLANDS: If construction activities include excavation or construction in delineated wetlands, the contractor shall consult with the base Natural Resource Manager in the 28 CES/CEIEC office at 2125 Scott Drive, (605) 385-2690. A map of delineated wetlands is maintained and available through the 28 CES/CEIEC office at 2125 Scott Drive, (605) 385-2690.
B.
WILDLIFE HABITAT: Construction activities including excavation or construction which has real or potential impacts to animal burrows, nests, feeding sites or other potential wildlife habitat shall be reported to the base Natural Resource Manager.
C.
LAND MANAGEMENT: Construction activities shall consider nonpoint source pollution control measures, provide for erosion control, water management, runoff disposal, landscaping and special soil problems.

ENVIRONMENTAL AWARENESS AND RESPONSIBILITY CHECKLIST
A.
If any of the environmental hazards listed on the form are encountered, the contractor shall call the telephone number shown on the form and then immediately notify the contracting officer of the situation.

1.13
EXCAVATION ENVIRONMENTAL RESPONSE PROCEDURES
A. Purpose – Beginning when EAFB was activated in 1942, burial or dumping was an industry accepted practice often used to dispose of materials no longer needed or wanted. Accidental spills also occurred periodically. Since the mid-1980s, EAFB environmental programs have been identifying, investigating, and remediating as necessary, waste disposal and spill sites. Unless specifically noted otherwise in the contract documents, there are no known waste disposal or spill sites that affect the specified work. However, during any grading or excavation there is the potential to uncover previously unknown waste or spilled material.
B. Unusual Material - Unusual material as used in this section shall mean any materials other than soils, rock or bedrock encountered during excavation that is not specifically identified as part of the work in the contract document. This includes but is not limited to: stained soils; free product fuels, oils, etc; suspicious odors; bottles, cans, drums, or other containers; wooden, metal, or cardboard boxes; rags, gloves, or personal protective equipment; trash or common landfill debris; concrete, concrete block, wood, metal, or other building materials and construction debris; or any other unidentified materials.
C. Response - If the contractor encounters any unusual materials during any excavation or grading, the contractors shall:
1.
Immediately stop the excavation or grading operation.
2.
Shut down the equipment and leave it in its present location.
3.
Do not attempt to determine the contents of any bottles, containers, boxes, etc.
4.
Withdraw personnel from the equipment and the excavation a distance of at least 50 feet upwind or cross wind or as directed.
5.
Contact the escort, inspector, contracting officer or the contracting officer’s designated representative immediately, or call 911.
6.
Remain in the area until released by the CO or designated representative.
D.
Every contractor and subcontractor employee shall be briefed on the response procedures in this section prior to beginning work on EAFB. Training shall be documented by the employee signing and dating a training log sheet. Log sheet shall be maintained on the immediate jobsite in the contractor’s project office and shall be available for inspection by the contracting officer or designated representative. Response procedures review shall be included in the contractor’s periodic safety meetings.
1.14 AFFIRMATIVE PROCUREMENT
A. Contractor shall comply with Ellsworth’s affirmative procurement program.
1. Contractor is required to purchase Environmentally Preferable Products manufactured from recycled and reclaimed goods. Environmentally preferable products are listed on the EPA’s Comprehensive Procurement Guidelines – Products (http://www.epa.gov/cpg/products.htm)
2. Exemptions to purchasing the listed products within the Comprehensive Procurements Guidelines – Products are as follows;
a. Item/Material not available within a reasonable period of time.
b. Item/Material fails to meet performance standards set forth in applicable specifications.
c. Item/Material is not available from a sufficient number of sources to maintain a satisfactory level of competition (item available from only one source)
a. Item/Material is only available at an unreasonable cost (the price of the recycled content product exceeds the cost of a comparable non-recycled product).
3. For each item considered on the Comprehensive Procurement Guidelines – Products list, contractor shall submit a materials Determination/Exemption form (provided at the end of this section) for approval.
B. Contractor shall submit a monthly diversion rate report. This report shall contain the total weight of material recovered for recycling or reuse, cost (estimated if necessary) to recover materials, and all receipts and documentation of recycling or reuse. The report shall also include the total weight of unrecoverable waste disposed off base, cost (estimated if necessary) of disposal, and documentation of disposal. Report shall be submitted to the Government no later than the 7th day of the following month (i.e. report for June is due by 7 July)
1.15 HAZARDOUS MATERIAL MANAGEMENT.
A. Contractor shall bring all hazardous materials intended for use on EAFB along with their Safety Data Sheets to the EAFB HAZMART to be labelled and entered into the EAFB hazardous material tracking system. The HAZMART hazardous material pharmacy can be reached at (605) 385-2692. Short-term contractors (those with a period of performance of six months or less) must have their hazardous materials labelled by HAZMART. Long-term contractors (those with a period of performance of more than six months) must have their hazardous materials approved via the base hazardous material approval system, Enterprise Environmental, Safety, and Occupational Health, Management Information System (EESOHMIS) and barcoded by HAZMART.

ASBESTOS: When encountering asbestos containing materials (ACM) contact the base Toxics Program Manager for inspection and handling. Do not disturb these materials unless you have a current worker/supervisor asbestos certification. The following are a few of the common types of ACM: 9 in. floor tile and mastics, hard pipe insulation, window putty, acoustic ceilings, some caulking mastics, transite siding or piping, and felt paper. PCB: When replacing or removing florescent light fixtures read the label on the ballast, if it says that this product does not contain PCB then it can be thrown into a dumpster. If the ballast label does not say anything about PCB then it may contain PCB and should be tested or treated as a PCB. Contact the Environmental Flight for further guidance. SANDING/DUST: Before sanding metals and painted surfaces or working in dusty areas contact Bioenvironmental Engineering. Painted surfaces may contain lead and other heavy metals which may create a significant health hazard. Excessive concentrations of nuisance dust may cause injury to mucous membranes. Bioenvironmental Engineering can determine what personnel equipment is required to prevent a health hazard. Call 28th MDG at (605) 385-1790.
