About this file

This document is a Past Performance Questionnaire (PPQ) for a federal contract opportunity. The PPQ is used to evaluate the past performance of an offeror being considered for the subject project, which is the Dental Expansion and Relocation Northern California VAMC Located on Travis AFB, California (Solicitation No. 36C77624R0085).

The PPQ requests information about the offeror's contract, including the contract title, number, type, location, award and acceptance dates, original and final contract prices, and a project description. It also evaluates the offeror's performance in areas such as quality, schedule, communication, management, cost control, and subcontract management. The evaluator provides an overall rating and indicates whether they would hire or work with the firm again. The PPQ instructions state that completed questionnaires should be provided back to the offeror to incorporate into their proposal submission.
