About this file
This Quality Assurance Surveillance Plan (QASP) outlines the government's systematic method for monitoring and evaluating contractor performance across multiple service and reporting requirements. The document establishes detailed performance standards and acceptable quality levels for various tasks, including reports (such as availability reports, sales reports, and marketing reports), inspections (like country of origin compliance, availability checks, and pricing verification), and deliveries. The Contracting Officer's Representative (COR) will conduct periodic and random inspections using different frequencies (daily, monthly, quarterly, bi-weekly) to ensure 95-100% compliance across different performance metrics.
The QASP includes specific consequences for unsatisfactory performance, such as potential contract termination, loss of future government contracts, and non-exercise of contract options. If performance falls below acceptable levels, the government may issue a Contract Discrepancy Report (CDR), requiring the contractor to respond within three days with explanations and corrective actions. For firm-fixed-price delivery orders, contractors are incentivized to provide quality products timely, as the government can require replacement or correction of work at no additional cost if contract requirements are not met. The document emphasizes comprehensive documentation, with all quality assurance records to be retained for the contract's duration and used in annual performance assessments.

QUALITY ASSURANCE SURVEILLANCE PLAN AND MATRIX
1. PURPOSE: This Quality Assurance Surveillance Plan (QASP) is a Government-developed and applied document used to make sure that systematic quality assurance methods are used in the administration of this performance-based service contract and in subsequent orders issued thereunder. The intent is to ensure that the contactor performs in accordance with performance metrics set forth in the contract documents, that the Government receives the quality of services and supplies called for in the contract, and that the Government only pays for the acceptable supplies received. The QASP will be used primarily as a tool to verify that the contractor is performing all services required by the PWS in a timely, accurate, and complete fashion.
2. SCOPE: The QASP is put in place to provide Government surveillance oversight of the contractor’s quality control efforts to assure that they are timely, effective, and are delivering the results specified in the contract or delivery orders. The QASP is not intended to duplicate the contractor’s management plan.
3. PERFORMANCE STANDARDS
a. Schedule - The due dates for deliverables and the actual accomplishment of the schedule will be assessed against original due dates and milestones established for the contract or delivery orders.
b. Deliverables – The deliverables required will be assessed against the specifications for the deliverables detailed in the contract and delivery orders for the required content, quality, timeliness, and accuracy.
c. Past Performance - In addition to any schedule and deliverable aspects of performance discussed above, pursuant to FAR 42.15, the Government will assess the contractor’s record of conforming to contract requirements and to standards of good workmanship, the contractor’s adherence to contract schedules including the administrative aspects of performance, the contractor’s history of reasonable and cooperative behavior and commitment to customer satisfaction, and the contractor’s business-like concern for the interest of the customer.
4. METHODS OF SURVEILLANCE: The primary methods of surveillance used to monitor performance of this contract will include, but not be limited to, random or planned sampling, periodic or complete inspection, and customer feedback. Each phase of the services rendered under the contract is subject to Government inspection during the contractor’s operations and after completion of a task or delivery. Inspections may be routine functions performed by the Contracting Officer’s Representative (COR), or performed by others outside of the command. The contractor shall not substitute Government inspection for effective quality control. Government inspections may be conducted in an unannounced manner by the COR or other individuals designated outside the organization.
5. PERFORMANCE MEASUREMENT AND QASP MATRIX: Performance will be assessed and measured as shown in the QASP Matrix below. If performance is within acceptable levels, it will be considered to be satisfactory. If not, overall performance may be considered unsatisfactory.

6. DOCUMENTATION: The COR will, in addition to providing documentation to the Contracting Officer, maintain a complete quality assurance file. All such records will be retained for the life of the contract. Information in the quality assurance file will be considered when completing the annual Contractor Performance Assessment Reporting System (CPARS) report.
7. INCENTIVES/DISINCENTIVES: The COR makes an annual report on contractor performance (CPARS or other annual report). The contractor’s failure to achieve satisfactory performance under the contract and/or delivery orders, reflected in the COR’s annual report, may result in termination of the contract and may also result in the loss of future Government contracts and/or task and delivery orders. The contractor’s failure to achieve satisfactory performance under the contract and/or delivery orders may result in the non-exercise of available options.
For each item that does not meet acceptable levels, the Government may issue a Contract Discrepancy Report (CDR). CDRs will be forwarded to the Contracting Officer with a copy sent to the contractor. The contractor must reply in writing within three days of receipt identifying the reason(s) for unsatisfactory performance and corrective action(s) to be taken, and how future occurrences of the problem will be prevented. Based upon the contractor’s past performance and plan to solve the problem, the Contracting Officer will determine if any further action will be taken.
For firm-fixed-price delivery orders, in accordance with the inspection provisions of the contract, the contractor will be incentivized to provide quality products in a timely manner since the Government can require the contractor, at no additional cost, to replace or correct work that fails to meet contract requirements.
