About this file
This document provides instructions for completing a Consolidated Maintenance Contract Inventory form used to track equipment and maintenance requirements at federal facilities, primarily DeCA (Defense Commissary Agency) locations.
The inventory tracking system requires completion of 16 data columns with specific formatting requirements. Core identifying information includes a three-letter Short Code combined with a six-digit Inventory Number (printed together without spaces or dashes), a two-letter Item Code, and a four-digit Object Class Code. Equipment details must capture the exact manufacturer name, model number, and serial number as they appear on data plates, along with the year of purchase in YYYY format. Physical location data requires the actual facility sub-location, with special notation for refrigerated units using case position numbers or CAN bus addresses. The form also collects preventive maintenance (PM) intervals measured in calendar weeks, and for refrigerant-containing equipment, the specific refrigerant type (F-gas designation such as R4449A or R744) and quantity in kilograms. A DeCA barcode (two letters and seven digits) is required for all accountable items, generally equipment only, with exceptions for displays and bakery dry items. When multiple identical items share a single inventory number, the quantity must be documented. Data entry standards specify using "Not Found" rather than "Unknown" or "Missing" when information cannot be located, and "Unreadable" when data is present but illegible. Text changes to red indicate items no longer at the facility for COR removal, while green text indicates new items or updates for COR review and SOMS entry. The COR provides inventory numbers for new items.
