About this file

This document is a Federal Supplement Exhibits file for the South Florida Regional Transportation Authority (SFRTA) containing required certifications, affidavits, and reporting forms for contractors and subcontractors participating in federally-funded projects. The exhibits establish comprehensive requirements for subcontractor participation, including the Schedule of Subcontractor Participation (Exhibit 1), which mandates that all proposers identify anticipated subcontractors with their trades, disciplines, addresses, registration numbers, and descriptions of work to be performed. The Monthly Subcontractor Utilization Report (Exhibit 6) requires contractors to document actual subcontractor participation monthly, including names, work descriptions, contract amounts, invoiced amounts, payment status, and completion percentages, with submission required alongside applications for payment. Additional documentation includes a Certificate of Disbursement of Previous Periodic Payment (Exhibit 7) certifying that all subcontractors and vendors have received their pro rata share of previous payments.

The file incorporates multiple federal compliance certifications applicable to awards of $25,000 or more, including Federal Tax Liability and Recent Felony Convictions Certification (Exhibit 8), Debarment and Suspension Certification (Exhibit 9) requiring System for Award Management (SAM) printouts, Restrictions on Lobbying and Non-Collusion Affidavit (Exhibit 14), and Contractor Acknowledgment (Exhibit 17). The supplement specifies 42 applicable federal provisions and clauses covering civil rights, disadvantaged business enterprises (DBEs), Davis-Bacon Act protections, Buy America requirements, environmental compliance, and drug and alcohol testing protocols. All certifications and affidavits must be executed by authorized representatives and, where applicable, notarized, with principals and first-tier subrecipients required to comply with federal debarment, suspension, and ineligibility standards and disclose any criminal convictions or civil judgments within the preceding three-year period.
