About this file

This is a Semi Annual Inspection Report for water-based fire protection systems at VA Grand Junction Building 6, completed on December 16, 2025, by James Trudeau of Platinum Fire Protection. The inspection was conducted in accordance with NFPA 25 2011 - Standard for the Inspection, Testing, and Maintenance of Water Based Fire Protection Systems and included a Joint Commission inspection.

The report identifies six deficiencies requiring corrective action. Deficiency #1 notes that two control valve signs are missing on control valves with locks or electrical supervision per section 13.3.2.2. Deficiency #2 indicates two gauges are due for replacement as they are not in good condition and not showing normal water supply pressure per section 5.2.4.1. Deficiency #3 documents that electrical waterflow alarm devices failed testing due to bad inspector's test valves that will open but do not want to shut after testing, allowing only manual trip operation per section 5.3.3.3. Deficiency #4 involves the basement inspector's test connection, which can only be manually tripped as the inspector's test valve is extremely difficult to close once opened. Deficiencies #5 and #6 identify missing signage on two basement backflow preventers (Backflow #1 and #2), both 3-inch butterfly valves. No deficiencies were found in the general wet system questions category. The inspector certified that all equipment tested was left in operational condition upon completion of the inspection except as noted.
