About this file

This is a Request for Information (RFI) log documenting 76 questions and responses for the 534-CSI-714 Cardiac Cath Lab 1 Site Prep project at a VA facility, with all entries dated May 26, 2026, and assigned to the Engineering department.

The RFI addresses critical construction and design clarifications across multiple disciplines. Key scope determinations include: signage limited to one new sign at Equipment Room AA205A plus two locations requiring signage insert changes (RME Storage AA204A and Sterile Supply AA207), with all signage sole-sourced through takeform; no lead or asbestos abatement in the base bid with stop-work procedures if materials are encountered; no blast-resistant components required; and confirmation that modular STARC walls are acceptable for ICRA requirements. Operational clarifications specify 4'-0" minimum clear hallway space preservation, ICRA barriers within defined zones while maintaining occupied corridors, possible concurrent phasing coordinated with VA Engineering, and after-hours work coordinated case-by-case. Infrastructure and equipment responses indicate HEPA negative air exhaust guidance provided in Appendix A; Siemens Desigo as the Building Automation System vendor; shop drawings available for Siemens and Philips equipment mounting; and freight elevator access via service elevators. Administrative requirements confirm Procore Project Management software acceptance for submittals and RFIs, VA badging required for all personnel with no limited notice to proceed provided, minimum two personnel required for GC Superintendent/SSHO/QC Manager roles, and CQC Manager qualifications may be met through construction experience with five years' relevant experience in lieu of formal degree requirements.

26-May-26 There is one new sign to be included in the scope of work.

That sign is located at the Equipment Room AA205A, see takeform information provided for this sign, there are two locations where the signage name will change and the signage insert will need to be provided, those locations are RME Storage AA204A and Sterile Supply AA206

3 During the site visit, I noticed many signs beyond Room-Identification in the area that will be receiving renovations including, "CAUTION-

BIOHAZARD", "Authorized Personnel Only", "Caution Radiation", "X-

Ray In use", "If you are pregnant or think you are pregnant, notify the

Radiology Technologist", and "What To Do In Case of Fire:". I do not see any call out for these types of signs in the Specifications or
