About this file

This document is an in-service inspection report for aboveground storage tank (AST) 8 located at the VA Medical Center in Martinsburg, West Virginia, completed on December 2, 2020. The tank is a 10,000-gallon UL-142 constructed concrete vault design manufactured by Convault Inc. in 2010, designated as State ID 002-00000189, and used to store diesel fuel at a fill height of 9.0 feet. The rectangular concrete vault tank measures 8.0 feet in width, 15.0 feet in length, and 9.0 feet in height. The inspection was conducted by certified inspector Ryan Peterson (API Certification No. 80187, STI Certification No. AC 44269) in accordance with West Virginia Code Chapter 22, Article 30, and Steel Tank Institute SP-001 6th edition standards using visual examination methods.

The inspection findings indicate the tank is fit for continued service. The concrete vault foundation, containment structure, and tank components are in satisfactory condition with no cracking or spalling observed. All required nozzles, valves, vents, fill basin, level indicators, and ancillary equipment (including digital transmitting capabilities for high level alarm, level gauge, leak detection, and flow tracking) are present and functional. The primary recommendation identified is cleaning and recoating of external nozzles and piping due to observed coating failure. Ultrasonic thickness (UT) measurements could not be performed due to tank configuration; these measurements are recommended during the next internal inspection to determine corrosion rate and future inspection intervals. Required next inspections per state regulations are: external inspection by December 2, 2023, with internal inspection and UT thickness testing to be tracked by the site.

1.0 EXECUTIVE SUMMARY

A Standard STI inspection was completed on Tank AST 8 in the VA Medical Center facility located at Martinsburg, WV on 12/02/2020 to meet the required annual and 3 year and annual inspection interval set fourth in the West Virginia Code: CHAPTER 22. ENVIRONMENTAL RESOURCES.ARTICLE 30, and to collect data in order to evaluate the mechanical integrity and fitness for continued service of the tank.

The tank is of a rectangular concrete vault design found to meet or exceed the minimum code requirements of STI SP-001 and the West Virginia code and regulations. This tank is of a UL-142 constructed concrete vault design, due to the configuration and design, no thickness measurements were taken on the tank or nozzles. These thickness measurements should be taken during the next scheduled internal inspection to determine corrosion rate and next inspection intervals. The visual inspection concluded that this concrete vault style tank is fit for continued service.

Tank external nozzles and piping have coating failure present. The tanks nozzles and piping should be cleaned and re-coated as needed to ensure protection against environmental corrosion.

Please refer to the results and recommendation sections of this report for other minor discrepancies and recommendations that were identified during the course of this inspection.
