About this file
This is an in-service inspection report for aboveground storage tank (AST) AST 3 located at VA Medical Center in Martinsburg, West Virginia, conducted on December 2, 2020. The tank is a rectangular concrete vault design manufactured by Convault Inc. in 2004, constructed to UL-142 specification, with an 8,000-gallon capacity divided into two sides for storing diesel fuel and gasoline. The inspection was performed in accordance with West Virginia Code Chapter 22, Article 30, and Steel Tank Institute SP-001 6th edition standards by certified inspector Ryan Peterson (API Certification No. 80187, STI Certification No. AC 44269).
The inspection concluded that the tank is fit for continued service. Key findings include: the concrete foundation meets NFPA 30 capacity requirements but exhibits minor cracking requiring sealing; the secondary concrete vault is free of cracking and spalling; tank nozzles are in satisfactory mechanical condition with appropriate valves and instrumentation; ancillary equipment includes digital transmitting capabilities for high-level alarm, level gauge, leak detection, and flow monitoring; and ventilation and required valves are in satisfactory condition. Critical deficiencies identified include: absence of tank grounding system requiring installation; minor cracking in the concrete foundation requiring sealing to prevent further degradation; and coating failure on external nozzles and piping requiring cleaning and recoating. Ultrasonic thickness measurements could not be performed due to tank configuration and should be conducted during the next internal inspection to determine corrosion rate. The next external inspection is due by December 2, 2023, with internal and UT inspections tracked by the facility.

1.0 EXECUTIVE SUMMARY
A Standard STI inspection was completed on Tank AST 3 in the VA Medical Center facility located at Martinsburg, WV on 12/02/2020 to meet the required annual and 3 year and annual inspection interval set fourth in the West Virginia Code: CHAPTER 22. ENVIRONMENTAL RESOURCES.ARTICLE 30, and to collect data in order to evaluate the mechanical integrity and fitness for continued service of the tank.
The tank is of a rectangular concrete vault design found to meet or exceed the minimum code requirements of STI SP-001 and the West Virginia code and regulations. This tank is of a UL-142 constructed concrete vault design built with a division plate to utilize two sides of the tank for gasoline and diesel storage and dispensary. Due to the configuration and design, no thickness measurements were taken on the tank or nozzles. These thickness measurements should be taken during the next scheduled internal inspection to determine corrosion rate and net inspection intervals. The visual inspection concluded that this split, concrete vault style tank is fit for continued service.
The tank grounding system was not present at the time of inspection and should be installed for continued use. The tanks diesel fuel pumping station had past issues with the flexible hose leaking, this seems to have been repaired in accordance with code standards and was found to be in satisfactory mechanical condition. The tanks concrete foundation has some issues with cracking. These areas should be sealed to prevent any further cracking and degradation due to the accumulation and freezing of rain water. Tank external nozzles and piping have coating failure present. The tanks nozzles and piping should be cleaned and re-coated as needed to ensure protection against environmental corrosion.
Please refer to the results and recommendation sections of this report for other minor discrepancies and recommendations that were identified during the course of this inspection.
