RFI Questions Submitted with Answers 36C24122R0135 Amendment 0015 405-13-104 Renovate Inpatient Ward, White River Junction, VT Questions #1 - #133 Appear in earlier amendments Question# 134 Please provide us the contractors contact information that handles the warranty and maintenance of the membrane roofing.
Answer: Scott Livernois with Garland; business phone 800-762-8225, ext 716; cell 603-998-9694; email: slivernois@garlandind.com.
Question# 135 Will the site visit attendance from 7/6/2023 be published.
Answer: Please see attached sign-in sheet.
Question# 136 Concerning the temporary project trailer:
1. Is there a specific source for electrical service to the trailer in either building 28 or 31?
2. Is there a location in building 28 or 31 that voice/data service for the project trailer can be served from?
Answer:
RESPONSE 136.1: There is a nearby electrical panel with enough spare capacity to power the construction trailer.
RESPONSE 136.2: Voice/data service for the project trailer can be served from the basement of building 31. Contractor will be responsible for installing cable and conduit from the demarcation point of the voice/data service located in basement of Building 31 to the job trailer.
Question# 137 Will any shutdowns of electrical panels need to be done during off hours, thus requiring premium time be included in the quote?
Answer: The bidders shall assume that all shutdowns of Critical Services will need to be performed during the least intrusive times, as determined by the owner for the conditions that exist at the time, and that may include “off-hours” under certain circumstances. However, active Nursing Units are generally better able to contend with the unintended consequences of service interruptions during daytime shifts when the facility is fully staffed.
Question# 138 Would like some clarity on the scope of work for the Nurse Call system based on the response to RFI question #83:
1. Will we only be responsible for furnishing and installing empty pathways for the Nurse Call system as shown in the bid drawings?
2. Will all the cost for removal and installation of Nurse Call system devices and cables be paid by the VAMC outside of this contract?
Answer: RESPONSE 138.1: The Contractor shall furnish and install required back boxes, conduit, junction boxes, pull chords, and electrical power in support of the nurse call system and shall integrate the activities of the separate contract Nurse Call System Vendor into the project schedule. All other costs related to the Nurse Call System installation shall be carried by the Owner.
RESPONSE 138.2: The VAMC will arrange and pay for the separate contract Nurse Call System Vendor to remove, protect, and hold for reinstallation all existing Nurse Call System components as shown on the record drawings. Any accessory materials that might remain after the separate contract Nurse Call System Vendor removal work shall be removed by the contractor. This may include back-boxes, conduit, and cabling.
Question# 139 Sheet 31-AS-101 shows an outline of the work area in the ground floor ceiling to support the layout of the first floor. Is there any expectation of encountering asbestos above ceiling on the ground floor? Sheet 31-AS-105A does not show any abatement requirements outside of the Ground Floor Nursing Unit.
Answer: It is likely that asbestos-containing materials (ACM) are present above the ground floor ceilings. Although the work to be conducted above the ceilings may be performed in proximity to ACM, only ACM discovered to be impeding the sanitary utility work will require abatement (e.g., obstructing ductwork). Identification and determination of what ACM, if any, will require abatement will be made after ceiling finishes are removed and the ceiling cavities are accessed. The removal of ground floor ceiling finishes to access the ceiling cavities below the first floor is not anticipated to require any asbestos abatement. Immediately upon removal of the involved ceiling area(s), and prior to commencing any other construction activities, the contractor shall notify the project HazMat consultant to investigate the existing conditions and determine ACM content. Where ACM is confirmed to exist the contractor, the HazMat consultant and the Owner shall agree upon a course of remediation and the cost of it. The remediation work shall be completed by the contractor based on a change order or other form of approval to proceed.
Question# 140 Please confirm that the VA will maintain “all-risk” property insurance on all existing property, both real and personal, throughout the duration of the project.
Answer: The Government does not intend to provide insurance to the Contractor. All contract insurance requirements are found under paragraph 4.9 SUPPLEMENTAL INSURANCE REQUIREMENTS (In accordance with FAR 28.307-2 and FAR 52.228-5).
Question# 141 Please confirm that the VA intends to provide “all-risk” builder’s risk insurance applicable to the renovation Work, and will the Contractor and Subcontractors of all tiers be included as insureds?
Answer: The Government does not intend to provide insurance to the Contractor. All contract insurance requirements are found under paragraph 4.9 SUPPLEMENTAL INSURANCE REQUIREMENTS (In accordance with FAR 28.307-2 and FAR 52.228-5).
Question# 142 If the VA intends to provide builder’s risk insurance for the renovation Work please confirm if the Contractor will be responsible for any deductibles and specify the amount.
Answer: The Government does not intend to provide insurance to the Contractor. All contract insurance requirements are found under paragraph 4.9 SUPPLEMENTAL INSURANCE REQUIREMENTS (In accordance with FAR 28.307-2 and FAR 52.228-5).
Question# 143 We will be cutting into the existing roof membrane. Can you tell me the manufacturer of the existing roofing system so that we can get a price from an installer of that roof membrane to maintain warranty?
Answer: See response to RFI #134.
Question# 144 How will the new network cabling, fiber optics, power etc., be brought into the ICU regarding time of day the work must be done, and is the cable or wiring to be installed in conduit?
Answer: Specification section 271500-2.3. B. 4 indicates fiber optic cable shall be installed in conduit. Coordinate with the owner for access times and reflect the same in the master schedule of activities.
Question# 145 See General Conditions Clause 4.29 IT Contract Security, paragraph 7. Liquidated Damages for Data Breach indicates the contractor is liable to the VA for liquidated damages in the event of a data breach or privacy incident involving any SPI the contractor/subcontractor processes. Please confirm the amount of the liquidated damages.
Answer: In the event that liquidated damages for data breach are required, liquidated damages would be applied in the amount of just compensation for the harm that is caused.

Question# 147 See Governmental Safety Requirements Part 1.7, please confirm if the Site Safety and Health Officer is required to be full time for this project?
Answer: All requirements of Solicitation part 1.7 “SITE SAFETY AND HEALTH OFFICER” (SSHO) AND “COMPETENT PERSON” (CP) apply to this contract. Note that the Site Safety and Health Officer shall maintain a presence on the site during construction operations.
Question# 148 Looking for further clarification on metals scope for this project. I’m seeing some reinforcement plates at CMU, and the original drawings from 1971. Can you clarify scope for structural steel erectors?
Answer: This request is not clear what is being requested. The solicitation documents stand as presented.
Question# 149 Please confirm subcontractors do not need to be registered in SAM in order to perform work.
Answer: All requirements of the solicitation apply. The VA requires the prime contractor to be registered in SAM; there are no requirements or clauses in this contract that require subcontractors to be registered in SAM.
Question# 150
4.10 - Clauses Incorporated by Reference state in part, ""This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. "" The section then lists 110 clauses. Please clarify which clauses are in effect, following the issuance of addenda 1-13+"

Question# 151 VAAR Clause 52.204-10 - Reporting Executive Compensation states in part," 2) First-tier subcontract information. Unless otherwise directed by the Contracting Officer, or as provided in paragraph (g) of this clause, [gross income from all sources the previous year was less than$300,000], by the end of the month following the month of award of a first-tier subcontract valued at or above the threshold specified in FAR 4.1403(a) on the date of subcontract award, the Contractor shall report the following information at http://www.fsrs.gov for that first-tier subcontract. (The Contractor shall follow the instructions at http://www.fsrs.gov to report the data.)". Please confirm if subcontractors with contracts valued greater than $300,000 are required to report executive compensation. Please also clarify what, if any, reporting requirements are in effect for subcontractors.
Answer: All requirements of the clause and this solicitation apply. This clause is specific to first-tier subcontracts.
Question# 152 Are subcontractors required to undergo a background check? If so, please provide required forms/details regarding the background check process & requirements.
Answer: All required personnel assigned to this project will undergo fingerprinting for issuance of contractor badging (copy of form attached). Photo badge requests require a background check by FBI. VA recommends project manager, project superintendent and lead for each subcontractor obtain a photo badge. All contractor staff who do not obtain a photo badge will be required to sign-in and sign out of FMS building daily to pick-up and return a contractor's badge.
Question# 153 Section 12 on drawing AS201 indicates new concrete infill between the existing slabs. The detail shows an angle in place to carry the infill, but no details. Is that angle and the structural support required to carry that infill existing?
Answer: This is a new condition created by removing a portion of the original exterior wall to provide for the communicating doorway. The exact field conditions that will be encountered are unknown but assumed based on original construction drawings.
