About this file

This document is a Present and Past Performance Questionnaire (PPQ) template for Solicitation HE125426QE045, issued by the Department of Defense Education Activity (DoDEA) for Safety Aides and Attendant Services in Mainland, Japan. The PPQ is designed to evaluate vendor performance on school bus transportation contracts and must be completed by vendors and returned by their references or customers to the Contract Specialist (Ms. Ronnie Swailes) and Contracting Officer (Mr. Dustin Fazzini) at DoDEA in Alexandria, Virginia.

The questionnaire contains two sections: a vendor information section requiring completion of contractor details (firm name, CAGE code, address, Unique Entity ID, contact information), work role designation (prime contractor, subcontractor, joint venture), project work percentage, and reference customer information (business name, address, contract number, final contract value, contract type, and period of performance); and an evaluation section to be completed by the vendor's customer using a rating scale (Poor/Unsatisfactory, Satisfactory, Good, Very Good, Excellent, or Neutral/N/A). The evaluation criteria assess contractor experience and performance across ten areas: transporting pre-K through high school students, special needs student transportation with safety aides, safety attendant services provision, personnel and vehicle management, incident and accident response, vehicle maintenance compliance with host nation laws, training provision and reporting, timely delivery of reports and contract deliverables, communication effectiveness, and overall performance. Respondents must provide comments for each rating and explain any problems and resolutions, along with a brief description of effort demonstrating relevancy to RFP requirements.
