About this file

This is an NFPA 72 System Record of Inspection and Testing Report for Kenosha County, Wisconsin, documenting the alarm inspection and testing of the fire alarm system installed at the Kenosha County Park & Ride facility located at 920 54th Street, Kenosha, Wisconsin. The inspection and testing was conducted by Siemens Industry, Inc., Smart Infrastructure Division on August 10, 2022, beginning at 12:00 PM and completing at 1:30 PM. The system under inspection is a Simplex 4010 fire alarm control panel (FACP) located in the parking garage elevator lobby. The testing included functional and visual inspections of the control unit, secondary power systems, notification appliance power extender panels, initiating devices, and interface equipment. All tested components passed inspection, including three heat detectors on floors one through three, one system smoke detector located above the FACP in the elevator equipment room, and three remote relay interface components for elevator recall functions. The system is powered by a 120-volt, 20-amp primary power supply with a sealed lead acid battery backup (manufactured April 30, 2019) providing 24 hours of standby capacity and 15 minutes of alarm mode capacity.

The inspection was performed by certified technicians Stanley Laitinen (License No. SL#150654) and Alexander Hanus (License No. AH#148912) in accordance with applicable NFPA standards. Notifications of the testing were made to building management, building occupants, and the monitoring organization (Alarm Detection Service) prior to and upon completion of testing. The smoke detector sensitivity testing passed in accordance with NFPA 72-2016 standards, and elevator recall functionality operated properly. The system was restored to normal operation at 1:30 PM on August 10, 2022. Acceptance of the test report was signed by Kenosha County representative Rhonda Haidinger on August 15, 2022. No equipment failures were identified during inspection or testing. The property owner is responsible for proper maintenance and care of the system, and Siemens retains the right to report any code deficiencies to the local Authority Having Jurisdiction if corrective actions are not completed within required timeframes.
