About this file
This is a Performance Evaluation Questionnaire issued by the Washington State Department of Enterprise Services, Division of Facilities Professional Service, Engineering and Architecture Services, for the Bellevue College Parking Garage Update project (Project No. 2025-728). The document is designed to collect performance information on General Contractor/Construction Manager (GC/CM) firms and their key personnel from previous clients who have worked with the proposing firms. The evaluation form requests that clients rate proposing firms and individuals across ten performance criteria on a scale of 1 to 10, with 10 indicating very satisfactory performance and willingness to hire again, and 1 indicating very unsatisfactory performance and unwillingness to rehire. Completed evaluations must be submitted to DES Project Manager Michele M. Hill at michele.hill@des.wa.gov no later than 2:00 PM PT on Wednesday, April 29, 2026.
The evaluation criteria assess the firm's ability to meet client expectations, demonstrate leadership with minimal state direction, deliver quality workmanship, manage project costs while minimizing change orders, maintain schedule adherence, maintain professionalism and prompt responsiveness, identify and minimize owner risk, complete construction closeout procedures, follow owner rules and safety requirements, and overall suitability for future engagement. Respondents may indicate if the referenced project involved multiple phases, high-rise construction, historic preservation, or occupied facility work. Individual evaluation scores will remain anonymous and confidential, with combined scores from multiple sources contributing to an overall performance rating for the firm or individual being evaluated.
