I would like to schedule a site visit to this location 1st week in June.

A virtual pre-proposal conference was held to discuss solicitation requirements.
See Attachment 19 for slide deck. After the short list has been made for Phase 2, we are anticipating an on-site visit.

With regard to the submittal of Key Personnel for Factor #2, and
Safety Information for Factor #3, please clarify whether or not this information is required only from the General Contractor, or are the Proposed Subs also required to submit the same information regarding their Key Personnel and Safety Records.
The prime should submit selected key personnel for all 5 positions who are subject to substitution requirements and ultimately responsible for their allocated role. It is expected key personnel attended weekly progress meetings. The prime may select internally or subcontractors for these roles. Submit only 1 person for each of the 5 roles, and/or indicate if someone will dual-hat. Note: dual-hat submissions should reflect qualifications for both roles.
Factor 3 safety submission is required of the prime contractor only.

The solicitation for this project references to two separate forms that need to be submitted in Phase 1. It states that these forms are attached; however, we cannot locate them. The project experience factor and past performance factor should work in tandem. The prime may submit up to 5 projects maximum for project experience.
Each project should have its own form.
The same project should be reflected in past performance. A prime may chose any combination of projects to get to 5, such as projects from the prime, or A/E projects, or subcontractor projects.
