GATE Well No. 4 June 27, 2022
Attendee
1.
I would like to schedule a site visit to this location 1st week in June. Can you advise who I should coordinate with?
A virtual pre-proposal conference was held to discuss solicitation requirements. See Attachment 19 for slide deck. After the short list has been made for Phase 2, we are anticipating an on-site visit.
2.
I downloaded the Sandy Hook project. I went through the package and could not find the bid sheet/checklist for submittal. Are you able to direct me to the correct part for this information?
Please refer to the Instructions section of the solicitation, which is Section L. That starts on page 40. You’ll want to pay special attention to pages 46-48 and 50-51 as it relates to detailed criteria for Phase 1. For Phase 1, do not submit any pricing information.
3.
Is it permissible to dual hat the on-site QC and Safety position?
It permissible to dual hat QC and Safety positions.
4.
Can site superintendent also perform safety manager duties and/or Quality manager duties?
The Site Superintendent may dual hat the Safety or Quality Control positions, if qualified. Note: one person cannot fulfill all three roles.
5.
Will A/E representative be approved to be quality manager?
An Offeror may submit their A/E for the quality position. Any candidate should have demonstrated quality control experience. Please review instructions and evaluation criteria for Factor 2.
Note: Offeror may not subcontract with draft design firm or subcontractors who produced the 35% plans.
6.
In Section H of the RFP The Key personnel are listed as the following:
PM, Job-site Superintendent, QC Manager, Safety Manager and DoR .
In Factor 2 Key Personnel on page 47 of 52 it states to Submit resumes for key personnel as identified in Section H of this solicitation and listed below, PM, Superintendent, On-site QC Specialist, On-site Safety Specialist and DoR.
Please confirm if for the QC and Safety Positions that you want resumes for the QC and Safety Managers not the On-site specialists.
Key Personnel have been revised in Section H and in Section L, Factor 2, so that they match; as follows:
· Project Manager
· On-site Superintendent
· On-site Quality Control Specialist
· On-Site Safety Specialist
· Designer of Record (NJ License)
Resumes should be submitted for all five positions.
7.
With regard to the submittal of Key Personnel for Factor #2, and Safety Information for Factor #3, please clarify whether or not this information is required only from the General Contractor, or are the Proposed Subs also required to submit the same information regarding their Key Personnel and Safety Records.
The prime should submit selected key personnel for all 5 positions who are subject to substitution requirements and ultimately responsible for their allocated role. It is expected key personnel attended weekly progress meetings. The prime may select internally or subcontractors for these roles. Submit only 1 person for each of the 5 roles, and/or indicate if someone will dual-hat. Note: dual-hat submissions should reflect qualifications for both roles.
Factor 3 safety submission is required of the prime contractor only.
8.
The solicitation for this project references to two separate forms that need to be submitted in Phase 1. It states that these forms are attached; however, we cannot locate them. Please direct us to where they can be found, or please upload/issue these forms for bidders use. They are: Under Section “L”:

Are all of the proposed Subcontractors to fill-out both of these forms?
The project experience factor and past performance factor should work in tandem. The prime may submit up to 5 projects maximum for project experience. Each project should have its own form. The same project should be reflected in past performance. A prime may chose any combination of projects to get to 5, such as projects from the prime, or A/E projects, or subcontractor projects. Please review the submission and evaluation criteria in sections L & M for those factors, as that is how the submissions will be evaluated.
