Mill North Slope Scaffolding and Deck Stabilization work plan:

Staff: Dates: Sept. 30-Oct. 4, 2019 Matt Smith (Project Lead), Tim George (Blasting Officer), Paul Hanis, and Marie Thorn.

Incident Response Lead: Greg Runyan

Goals:

1. Ensure the health and safety of all members of the team.

2. Protect all workers from heavy metals exposure.

3. Identify potential explosives before any disturbance takes place.

4. Be prepared in the case of an explosive event or other injuries.

5. Mitigate any explosives discovered.

6. Install 8 scaffold pads (4x12x24 nom) along north wall of Mill.

7. Install 4-5 pads (4x12x24 nom) for deck stabilization.

Process and Timeline:

1. Discuss the plan with the team. Allow comments and concerns to be addressed and ensure that all members are comfortable moving forward with the plan.

2. Complete a GAR/SPE/ORMA model and review applicable JHAs (winch, explosives).

3. Tim will be the overall safety office and will keep the “big picture” view unless a potential explosive is discovered. In this case Matty will transition to this role while Tim assesses the hazard.

4. Establish an anchor approximately 100 feet from the building that will be used to attach an electric winch to. This creates a safe working distance from possible explosion.

5. Use appropriate rigging to move any heavy woody debris that is within the work area. Smaller debris will be moved by hand. Only move as far as necessary.

6. Establish pads for scaffold tower to be erected by Phase III contractor in 2020. Eight pads will be installed using 4” x 12” x 24” pressure treated wood.

7. Layout of pads will be 16” from wall plumb line, 7 feet between frames that are 5 feet wide.

There will be 4 sets of frames used so the total run will be 21 feet x 5 feet wide. Slope angle is approximately 43°.

8. Establish 4-5 pads for deck stabilization.

9. Soil will be dug only as deep as necessary to fully support each 4 x 12 pad.

10. Utmost care will be taken when excavating the soil in order to not disturb any buried blasting caps. No compression of soil will be allowed. Water will be used to help settle and compact the pad areas.

11. Layout of pads will be confirmed and each pad will be checked for level in two directions.

12. After Action Review will be conducted to identify if our plan was sufficient to meet all of our goals and identify if any changes need to be made next time this type of activity is done.

PPE and Engineering Controls:

• A temporary waterline will be established by tying in to the main waterline near the water treatment house and run up past the top of the Mill Building and down the northern slope.

• A decontamination facility will be located near the top of Mill.

o Canopy for weather, if needed.

o Garbage cans and liners o Wet wipes o Duct tape o Water buckets and Hudson sprayer for decontamination.

• Half-face Respirators with combo Mercury vapor/particulate cartridges.

• Tyvek suits with hoods

• Helmets while winching

• Eye protection, face shields

• Heavy duty clothing such as insulated Carhartt-type bibs and coats.

• Leather gloves for winch/cable work

• Rubber gloves for digging

• Rubber boots with safety toes.

Rescue gear for potential steep angle rescue will be staged near top of Mill prepared for quick deployment. Greg Runyan in charge of rescue Ops.

Schedule:

Sept. 30:

• Tim drives to Kennecott.

• Run waterline to site. Hose down soil/set up sprinkler.

• Stage materials for deck pads, shoring and scaffold pads at top of Mill.

• Stage winch, generator, rigging materials at top of Mill.

• Photo document site before and artifact or ground disturbance.

• Tim leads explosives awareness training for project crew (1 hour?)

October 1:

• Explosives ID briefing led by Tim.

• Last minute staging and prep.

• Review plan and get everyone’s approval.

• Commence work, plan Lunch and other breaks.

• Stop all work by 4:30 to allow time to decon and shower.

• Re-asses plan for following day’s activities.

October 2:

• Review previous day’s activity and address additional concerns.

• Complete project or plan third day??
