About this file
This is a Past Performance Information Summary template (Part B) for the TEIS IV On-Ramp contract opportunity solicitation W91RUS27RA001 issued by the Department of the Army Materiel Command Army Contracting Command Aberdeen Proving Ground. The template requires offerors to submit detailed past performance references demonstrating relevant experience on prior contracts.
Offerors must complete a separate summary for each reference contract submitted, providing comprehensive information organized into four sections. Section 1 requires contract identification details including offeror name, CAGE code, UEI number, contracting agency, contract number, contract type, period of performance, original and current contract values, and primary customer points of contact (Contracting Officer and Contracting Officer's Representative). Section 2 addresses performance evaluation, requiring offerors to describe unique technical areas, demonstrate how prior contract experience applies to applicable sample tasks from the Technical Approach Factor, and disclose any past or present litigation, administrative proceedings, cure notices, show cause notices, or contract terminations with explanations of corrective actions taken and problem severity. Section 3 requires offerors to provide the same detailed information for any proposed major subcontractors. Section 4 permits new corporate entities to submit data on prior contracts involving their officers and employees, provided they discuss the specific roles performed by such individuals. The government reserves the right to contact references and verify information, making it the offeror's responsibility to ensure all reference data is current and accurate for evaluation purposes.
