About this file

This is a DD Form 1423-1, Contract Data Requirements List (CDRL), which is a federal contracting template used to specify data items that contractors must deliver as part of contract performance.

The form contains one data item requirement identified as TS24 for a "Site Survey" deliverable. The contract reference is Sample Task 2 para 3.1.1.1, 3.2.1, with the requiring office listed as HQUSAISEC EUE-ISE-TS. The site survey report must be submitted in draft form within 5 business days after each trip completion and in final form within 5 business days of receiving comments from the Technical Monitor (TM). If comments are not provided within 10 days of draft delivery, the draft automatically becomes the final deliverable. The report must be developed in Microsoft Word format and submitted in soft copy to both the TM and Contracting Officer's Representative (COR) via email. Distribution requires one copy each to the TM and COR. All travel must comply with Joint Travel Regulations (JTR), and only one site survey report is required when multiple personnel support the same task order.

The site survey report must clearly document facility condition, identify issues or problems requiring consideration for successful project implementation, include photos of potential problems, and provide accurate point of contact information for survey participants and key site personnel with organizational affiliations, phone numbers, and SMO codes. The report format requirements specify 8.5" x 11" portrait-oriented letter-size paper with Arial, size 12 font, grammatically correct content, minimum one page with a typical maximum of five pages. The cover sheet must display USAISEC and CECOM logos and include the survey title with location and project, date of preparation, directorate information, disclaimer statement, preparer signature block, and government reviewer information. Subsequent sections must include location and travel dates, purpose, traveler names, points of contact from participating organizations, discussion of gathered information relevant to project completion, potential impacts, recommendations, and conclusions. The form indicates an estimated total price of 2 with 2 draft copies and 0 final copies required for delivery.
