About this file
This is a Past Performance Information Summary template (Part B) for federal contract proposals, specifically designed for submission to the Total Engineering and Integration Services (TEIS) IV On-Ramp opportunity under solicitation W91RUS27RA001 with the Department of the Army.
The template requires offerors to provide detailed past performance data for each reference contract, organized into four sections. Section 1 requests contract description information including the offeror's name, CAGE code, DUNS number, program title, contracting agency, contract number and type, period of performance, original and current contract values, completion dates, and primary customer points of contact (Contracting Officer and Contracting Officer's Representative). Section 2 requires performance narratives addressing technical areas unique to the contract, application of experience to relevant sample tasks outlined in the Technical Approach Factor, disclosure of any litigation or administrative proceedings with results or current status, provision of cure and show cause notices if applicable, discussion of contract terminations, and analysis of problems encountered and corrective actions implemented. Section 3 mandates that offerors provide the same information for any proposed major subcontractors. Section 4 allows new corporate entities to submit data on prior contracts involving their officers and employees, with required discussion of those individuals' specific roles. The template emphasizes that offerors should limit submissions per Section L-19 Table L1 constraints and that the government reserves the right to contact references directly to verify performance information.
