3.2.4 Activity Hazard Analysis (AHA) ;
Before beginning each activity, task or Definable Feature of Work (DFOW) involving a type of work presenting hazards not experienced in previous work activities, the Contractor(s) performing that work activity must prepare an AHA. AHAs may be developed by the Prime Contractor, a sub-contractor, any sub-tier contractor, or supplier performing the work and shall be reviewed and approved by the prime contractor’s TOSM before submitting to the GDA for review and acceptance. The TOSM shall add their electronic signature to the AHA prior to submitting to the FUELS Safety Team.
Section 01 35 26 Page 7
Format the AHA in accordance with Chapter 1-6 of EM 385-1-1. Specific AHA requirements are identified in paragraph 6 of each chapter of the EM 385-1-1
NOTE: Regarding pre-accepted AHAs, the Government reserves the right to require the Contractor to revise and resubmit any AHA or require a new AHA be developed if the AHA(s) fail(s) to effectively identify the work sequences, i.e., numbering job steps, specific anticipated hazards, site conditions, and control measures to personnel to include the qualifications described in EM 385-1-1 Chapter 2-6.
Each employee performing work as part of a work activity, job or DFOW identified in a Service Order must review the AHA(s) for that work and sign a signature log specifically maintained for that AHA(s) prior to starting work. The TOSM shall review and maintain copies of signature logs of job site reviewed AHAs. Provide copies to GDA upon request. The SHO (L1, L2, or L3) onsite shall use the AHAs during daily inspections to ensure the implementation and effectiveness of the required safety and health controls for that work activity.
Prime Contractor shall review and update the AHAs every 12 months throughout the life of the contract. The review should be performed in conjunction with the APP review. If no updates are required to an AHA, a letter from the prime contractor to the CEHNC Project Manager in RMS must be provided to this effect. If updates are made, resubmit only those AHAs updated to the CEHNC Project Manager via a letter along with the applicable AHA(s) from the prime contractor in RMS. The FUELS Safety Team will review and provide acceptance through RMS.
Utilize the list shown in Table 8-1 below for the minimum required AHAs to be included in the APP. These AHAs shall be submitted to the KO within 30 calendar days following contract award. The Fuels Safety Team shall have between 21 and 30 calendar days to review and provide a response in RMS. Once AHAs are accepted with a Code A in RMS, they will be deemed pre-accepted AHAs to be used throughout the life of the contract with the exception of those identified in Section 3.2.5 and the AHA, Electrical - Inspect, Troubleshoot, Remove & Replace and/or Repair found in PWS Appendix C, Section 01 35 26, Paragraph 8.0, which shall be task specific or as directed by the GDA.
NOTE: Do not restate the safety requirement(s) listed in EM 385-1-1 or OSHA. The AHA must identify the specific JOB STEPS, i.e. numbering 1, 2, 3, etc., potential HAZARDS in performing each Job Step and the detailed CONTROLS to abate the hazards”. The focus shall be on by “Who,” “When” and “How” the control will be achieved.
EXAMPLE: If a control for a hazard requires PPE, specify the PPE to be worn to perform the work activity. Do not state, appropriate PPE will be worn. The Control shall be identifiable and specific to abate the stated hazard, e.g., state the type of gloves, eyewear, footwear, respiratory and/or hearing protection, clothing, etc.
Once pre-accepted, AHAs will not require additional review at the Service Order level except as noted. If a work task is not identified the prime contractor is responsible to submit a new AHA(s) for review and acceptance. A new AHA(s) may be added throughout the life of the contract once direction is given from the FUELS Safety Team to add to the pre-accepted listing.
Note. For a non-mandatory formatted outline of an AHA see ENG Form 6206 (Activity Hazard Analysis) (See Chapter 1-10.b.)
Section 01 35 26 Page 8
3.2.5 AHA Change Management
If site conditions or work activities change, field changes are acceptable by “redlining” the AHA as follows:
A. This AHA shall be reviewed and modified as necessary to address changing site conditions, operations, work activities or change of Competent Person(s)/Qualified Person(s).
B. The AHA(s) requiring minor change(s)/addition(s), i.e. < 33% of any AHA is “redlined,” or if the overall RAC does not change to an AHA(s) that were submitted with the SO request, “redlining” is acceptable.
C. Changes or updates to an AHA that do not increase the RAC are not required to be resubmitted for acceptance by the GDA.
D. If the AHA(s) requires moderate changes/additions, i.e., > 33% of any AHA is “redlined,” and the initial RAC increases due to a changes made to an AHA, the AHA shall be resubmitted to the GDA, i.e., FUELS Safety Team, for review and acceptance prior to work proceeding.
E. If the site conditions or work activities change requiring the submission of a new or pre-accepted AHA, it shall be submitted to the FUELS Safety Team for review and/or acceptance prior to the work commencing.
F. Change of Competent Person(s)/Qualified Person(s) require prior approval of the TOSM.
G. Workers/crews shall have in their possession the current AHA(s) that reflect current site conditions, personnel, equipment, and control measures, etc. while the work is being performed.
H. All new or pre-accepted AHAs that were not submitted with the original SO request shall include the Service Order Desk email address so the documents can be appended to the appropriate portfolio.
3.2.6 AHAs and site-specific plans for activities having higher “Risk Factors”
If/when the Contractor is tasked to perform work which has higher risk factors, they shall submit job specific AHAs and/or/plans as specified in EM 385-1-1. Work activities having higher “Risk Factors” that could lead to imminent danger situations and/or conditions necessitating closer assessment of the work being recommended will require site specific preparations. These activities shall be carefully planned and job/activity specific AHA(s) developed to ensure the residual risk level RAC is no greater than a Medium Risk. The AHA(s) and required plans/procedures for these work activities shall be submitted to the FUELS Safety Team for review and acceptance prior to work being performed. As part of the prime contractor’s planning, these new and/or updated AHAs should be submitted with the Service Order request to preclude delays in last minute reviews. Specific AHA requirements are identified in paragraph 6 of each chapter in the EM 385-1-1. See Higher Risk Factor Activities list below. The list is not all inclusive.
Section 01 35 26 Page 9
NOTE: Regarding the SSA and Sections from the APP, those documents may be provided to prime contractor’s subcontractor for review and use; but this should not be necessary since the contract requirements has a provision that the accepted APP shall be provided to each subcontractor for compliance.
Higher risk factor activities are as follows:
• Wearing of Respiratory Protection (not included in a pre-accepted AHA)
• Lead Abatement
• Asbestos Abatement
• Energized Work
• Standard and Critical Lifts
• Fall Protection (not included in a pre-accepted AHA)
• Demolition/Renovation (large scale such as demolishing a building or structure)
• Excavation/Trenching (5’ or greater in depth)
• Confined Space Entry
• Abrasive Blasting
• Dive Operations
The contractor is required to have a L1 who shall have no other responsibilities, for maintenance or repair being performed on these higher risk factor activities. If the TOSM determines that a L1 is not necessary, this determination shall be provided to the GDA in the Service Order for acceptance. Example: For many standard lifts, an L3 may be utilized but the determination rests with the TOSM. The above list is not meant to exclude additional hazardous work activities that may have a significant risk and does not relieve the contractor from maintaining an acceptable level of safety, in accordance with the EM 385-1-1.
NOTE: When pre-accepted AHAs are used for a Service Order a statement will be provided on the Service Order email stating, “The attached pre-accepted AHAs are required for this task. No new AHA is required for this task, therefore pursuant to the PWS, no USACE Safety review is required.”
