0008Task 6b: Additional Water Tightness TestingThe work for this item shall include the equipment, materials, and labor necessary for pre-grouting, redrilling, and additional watertightness testing of the primary grout zones + 5 feet of any anchor type (CBZ, underream, and belled anchors (if the initial water tightness testing fails)) prior to anchor installation. 0009Task 8: Reinforced Concrete Anchor Pad ConstructionThe work for this item shall includes activities necessary for all work related to the placement of reinforced concrete for anchor pad construction. This item inclues the cost of all labor, testing, equipment and tools, materials and supplies required to complete the reinforced concrete work under this contract including excavating, furnishing, forming, placing, curing, and finishing the concrete, including cement, reinforcing and joint materials as needed.12EA
0010Task 9: Anchor Bar Type, Fabrication, and Anchor System ComponentsThe work for this item shall include the equipment, materails, transportation, and labor necessary to fabricate and procurement the anchor system components for onsite delivery of the anchor bar and anchor system compoments in accordance with the PWS. 0011Task 12: Anchor Field TestingThe work for this item shall include the equipment, materials, and labor necessory for in-situ field setup, reporting and field load testing in accordance with the PWS requirements. All bar anchors must have tension applied Verification tests in accordance with FWHA NHI 05-039 or (Modified) Extended Creep Test in accordance with PTI DC35.1-14 as required in the PWS. No more than three anchors may have modified extended creep tests. Verification and Modified Extended Creep Tests schedules are provided in Attachment II.1JobXXXX
0012Task 13: Anchor Long-Term MonitoringThe work for this item shall include but not be limited to all labor, equipment, materials, testing, calibration and performing all operations in connection with furnishing, transporting, and installing all instrumentation, data collection and monitoring equipment as specified in the PWS.1JobXXXX
0013Site RestorationThe work for this item shall include but not be limited to labor, equipment, materials, and performing all operations in connection with furnishing, transporting, and placing top soil, subgrade preparation, seed, mulch, fertilizer and other requirements included in the PWS.1JobXXXX
0014Work SubmittalsThis item includes the materials and labor necessary for preparation of all work submittals including pre-fieldwork submtitals, contract admistration, and the Anchor Field Installation and Testing Report.1JobXXXX

NOTES:

1. Prices must be entered for all line items in the schedule. Additions will be subject to verification by the Government. In case of variation between the unit prices and the total amount, the unit price will be considered the price submitted.

2. A modification to a quote which provides for a single adjustment to the total amount submitted, should state the application of the adjustment to each respective lump sum price affected. If the modification is not so apportioned, the single adjustment will be applied to Item Nos. 0001 thru 0014.
