ATTACHMENT K - SMALL BUSINESS COMMITMENT DOCUMENT

Other than small businesses (SB) Offerors are required to complete a Small Business Commitment Document (SBCD). The Offeror shall propose the level of commitment of SB concerns (e.g., Small Business (SB), Small Disadvantaged Business (SDB) (including ANCs and Indian Tribes), Woman- Owned Small Business (WOSB), Historically Underutilized Business Zone (HUBZone), Veteran-Owned Small Business (VOSB), & Service-Disabled Veteran-Owned Small Business (SDVOSB)) in the performance of this acquisition relative to the objectives/goals set-forth in the evaluation of this factor.

A Minimum Quantitative Requirement (MQR) Total Small Business Commitment of 12% of the total contract value is assigned to this acquisition.

Note: Pursuant to the Small Business Act, Section 8(d), a business is considered small for Government procurement if it does not exceed the size standard for the NAICS code that the prime contractor believes best describes supply/service being acquired. In other words, the size of the prime’s supplier is determined by the applicable NAICS code of their joint-venture, teaming, partnership, or subcontract, which may or may not be the same NAICS code as the one for the prime contract with the Government

1. Indicate the total contract dollar value and percentage of work based on total contract value of the acquisition to be performed by both large and SB (include the percentage of work to be performed by both the prime and both large and small business subcontractors):

For example: If the LB Prime proposes a price of $1,000,000 (including all options), and SB will provide $250,000 in supplies/services as a subcontractor, the %-age planed for SB is 25%; and 75% for large business equating to 100%.

Total percentage planned for Large Business(es)

Total Percentage planned to be self-performed by Prime _____________$_______ = ______%

Total percentage planned for Small Business(es)

Total: 100%

2. Indicate the total combined dollar value and percentage of work based on total contract value of the acquisition to be performed by each SB category. The percentage of work performed by any SB that qualifies in multiple SB categories should be counted in all applicable categories.

For example: Subcontractor A (a VOSB, SDVOSB, and WOSB) performing 2%; and Subcontractor B (a SDB, HUBZone, and WOSB) performing 3%. The results will equate to SB 5%, SDB 3%, HUBZone 3%, WOSB 5%, SDVOSB 2%, and VOSB 2%).

Small Disadvantaged Business (SDB) (5% suggested goal) $ % Woman-Owned Small Business (WOSB) (1% suggested goal) $ % Historically Underutilized Business Zone (HUBZone) .5% Suggested goal)

Veteran-Owned Small Business (VOSB) (1% suggested goal) $ %

Service-Disabled Veteran-Owned SB (SDVOSB) (.5% suggested goal)

3. Identify the Prime Offeror and type of supply/service that the Prime will provide. Then list each type of subcategory small business subcontractor and the principal supply/services to be provided by the SB concern. Provide the UIC and/or CAGE code for the Prime and each planned subcontractor. Also, provide the anticipated NAICS code(s) that the Prime believes best describes the supply/service being acquired by the subcontracts with each named subcontractor. SB subcontractors that qualify in multiple SB categories should be listed under each applicable small business category.

Example: If a SB concern Company A qualifies as a WOSB and a SDVOSB, the Offeror can add Company A info to four of the SB categories below: (b) SB, (d) WOSB, (f) VOSB, and (g) SDVOSB

(a) Prime Offeror:

Name of Company Identified (include UIC and/or CAGE Code)

Type of Supply/Services (include NAICS Code for Subcontractor)

Note, adjust/modify format for information box, if applicable.

(b) SB Concerns

(c) SDB Concerns (including ANCs and Indian Tribes)

(d) WOSB Concerns

(e) HUBZone Concerns

Name of Company Identified (include UIC and/or CAGE Code)

(f) VOSB Concerns

(g) SDVOSB Concerns

4. Describe the type and extent of commitment to use small business concerns (e.g., what types of commitments and of what duration, if any, are in place for this specific acquisition - written contract, verbal, enforceable, non-enforceable, joint-venturing, mentor-protégé, etc.) Written, enforceable commitments are weighted more favorably and should be signed by both parties. Provide documentation regarding commitments to small business for this effort. If written commitment, then a copy of the agreement must be attached. Copies of such agreement will not count against the proposal page limitation.

Company” Type of Commitment (include duration):

5. Small Business Utilization Past Performance – Submit past performance information reflecting the Prime’s past success in meeting its small business commitment for each project listed by the Offeror under Factor 1, Past Performance. If the project was performed for a Federal agency, all Individual Subcontract Reports (ISRs) and Summary Subcontract Reports (SSRs) from the Electronic Subcontracting Reporting System (eSRS) for the project must be submitted. Copies of ISR’s and SSR’s will not count against the proposal page limitation.

If project not performed for a Federal agency, for each project submitted under Factor 1, Past Performance, please submit the following:

(a) Name of the project & location.

(b) Total dollar amount of project and percentage of the contract’s total value self-performed.

(c) Total dollar amount and percentage of the contract’s total value that was subcontracted.

(d) Total dollar amount and percentage of the contract’s total value subcontracted to small businesses.

(e) Total dollar amount and percentage of the contract’s total value subcontracted to other than small businesses.

In addition to the above project submission information, the Government reserves the right to review Offerors Individual Subcontract Reports (ISRs) and Summary Subcontract Reports (SSRs) from the Electronic Subcontracting Reporting System (eSRS).
