If a preparatory inspection is held during the day while an alternate shift is underway, the notes of the preparatory inspection shall be read during the immediate next shift. Any questions or uncertainties shall be brought to the primary shift for resolution. If an alternate shift has begun after the 'definable feature of work' is already underway, the preparatory meeting minutes shall be read during the first alternate shift. Secondary preparatory meetings shall be conducted for shifts not present at the initial meeting. As a minimum, foreman shall attend these secondary meetings.
3.8.2 Initial Phase

Indicate the exact location of initial phase for future reference and comparison with follow-up phases.
h. The initial phase should be repeated for each new crew to work onsite, or any time acceptable specified quality standards are not being met.
i. Initial inspections shall typically take place between 6:00 AM and 6:00 PM, unless the 'definable feature of work' must be completed
SECTION 01 45 00.00 10 Page 12 outside these hours. If an initial inspection is held during the day while an alternate shift is underway, the notes of the initial inspection shall be read during the next shift. Any questions or uncertainties shall be brought to the primary shift for resolutions.
If an alternate shift has begun after the 'definable feature of work' is already underway, the initial meeting minutes shall be read during the second alternate shift.
3.8.3 Follow-up Phase
Perform daily checks to assure control activities, including control testing, are providing continued compliance with contract requirements, until completion of the particular feature of work. Record the checks in the CQC documentation. Conduct final follow-up checks and correct all deficiencies prior to the start of additional features of work which may be affected by the deficient work. Do not build upon nor conceal non-conforming work.
3.8.4 Additional Preparatory and Initial Phases
Conduct additional preparatory and initial phases on the same definable features of work if: the quality of on-going work is unacceptable; if there are changes in the applicable CQC staff, onsite production supervision or work crew; if work on a definable feature is resumed after a substantial period of inactivity; or if other problems develop.

d. Verify that recording forms and test identification control number system, including all of the test documentation requirements, have been prepared.
e. Record results of all tests taken, both passing and failing on the CQC report for the date taken. Specification paragraph reference, location where tests were taken, and the sequential control number identifying the test. If approved by the Contracting Officer, actual test reports may be submitted later with a reference to the test number and date taken. Provide a copy of tests performed by an offsite test
SECTION 01 45 00.00 10 Page 13 facility directly to the Contracting Officer. Failure to submit timely test reports as stated may result in nonpayment for related work performed and disapproval of the test facility for this contract.

Once this is accomplished, notify the Contracting Officer that the feature is ready for the Government Pre-Final Inspection.
3.10.2 Pre-Final Inspection
The Contracting Officer will perform the pre-final inspection to verify that the facility is complete. A Government Pre-Final Punch List may be developed as a result of this inspection. Ensure that all items on this list have been corrected before notifying the Contracting Officer, so that a Final Inspection with the customer can be scheduled. Correct any items noted on the Pre-Final Inspection in a timely manner. SECTION 01 45 00.00 10 Page 15
Indicate a description of trades working on the project; the number of personnel working; weather conditions encountered; and any delays encountered. Cover both conforming and deficient features and include a statement that equipment and materials incorporated in the work and workmanship comply with the contract. Furnish the original and one copy of these records in report form to the Contracting Officer daily within 24 hours after the date covered by the report, except that reports need not be submitted for days on which no work is performed. As a minimum, prepare and submit one report for every 7 days of no work and on the last day of a no work period. All calendar days shall be accounted for throughout the life of the contract. The first report following a day of no work shall be for that day only. Reports shall be signed and dated by the CQC System Manager. Include copies of test reports and copies of reports prepared by all subordinate quality control personnel within the CQC System Manager Report.
3.12 DEFICIENCY TRACKING SYSTEM
The Contractor shall maintain a cumulative list of deficiencies identified for the duration of the project. Deficiencies to be listed include those identified by the Contractor's Quality Control observations, test failures, Government oral observations and Notifications of Noncompliance. As a minimum, the list shall include the information contained in the Deficiency List included in the Attachments to Specifications. The list shall also contain items that pose the potential to cause deficiencies and shell require additional action or in-depth analysis by the Contractor as deemed appropriate to the Contracting Officer to ensure contract requirements are met (e.g. verticality inaccuracy leading to potential overlap deficiency). A current copy of the list shall be maintained at the project site at all times and shall be made available for review by Government personnel. Copies of updated listings shall be submitted to the Contracting Officer at least every 30 days. Payment will be withheld for deficient work until it has been corrected.
3.13 CALIBRATION PLAN
Contractor shall provide Contracting Officer with a plan for calibration of any instruments used. Calibration plan shall be based on manufacturer's recommendations and project requirements. Contractor shall supply as part of calibration plan a copy of manufacturer's calibration certificates, instructions, and recommended intervals of calibration. The calibration plan shall be received 30 days prior to use of the instrument and shall be approved by Contracting Officer prior to use. The calibration plan shall be included in the Contractor's Quality Control Plan (CQCP). Each time an instrument is calibrated, an updated calibration form shall be provided by the calibration agency conforming to the all manufacturing recommendations.
3.14 SAMPLE FORMS
Daily Contractor Control (CQC) Reports and Monthly Contractor Control (CQC) Reports shall be submitted to the Contracting Officer. A sample Contractor's Quality Control Report form is included in Volume 3 - Attachments to Specifications.
3.15 NOTIFICATION OF NONCOMPLIANCE
The Contracting Officer will notify the Contractor of any detected noncompliance with the foregoing requirements. The Contractor shall take
SECTION 01 45 00.00 10 Page 16 immediate corrective action after receipt of such notice. Such notice, when delivered to the Contractor at the work site, will be deemed sufficient for the purpose of notification. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to such stop orders will be made the subject of claim for extension of time or for excess costs or damages by the Contractor.
