About this file
This document contains a federal contract opportunity notice and a process flow diagram for health readiness services. The U.S. Army Contracting Command - New Jersey, on behalf of the Defense Health Agency, is seeking health readiness services including immunizations, physical examinations, mental health assessments, dental services, and periodic health assessments. The services will support reserve components of the Army, Air Force, Navy, Marine Corps, Coast Guard, active components, and Defense civilians. The contractor must provide these services through a nationwide network of clinic providers, a call center, and at designated service member sites. The government intends to award a single five-year indefinite-delivery, indefinite-quantity contract with both fixed-price and cost-reimbursement task orders under North American Industry Classification System code 621112. The anticipated solicitation release date is September 30, 2017, and interested parties should monitor the Federal Business Opportunities website.
The process flow diagram depicts the periodic health assessment process. A service member completes a self-assessment which is reviewed by trained healthcare personnel. As needed based on the self-assessment responses, the service member undergoes a mental health assessment with a provider. The healthcare provider then reviews all information, conducts any required assessments, and makes referral recommendations.
