A. The entire submission, MATOC and SEED project, is required to be submitted as a hard copy and on a CD.

TO:

1. Does the complete and final pricing need to be included on the CD submitted or will the hardcopy take precedence?

A. The pricing for the SEED project is to be submitted as a hard copy and not included on the CD.

Change Amendment 4, Block 14, Sentence 2 -

FROM:

2. Performance, Payment and Bid Bonds are required for the seed project #542-18-107 Flooring Replacement Phase 1.

TO:

2. Performance and Payment Bonds are NOT required with submission of seed project #542-18-107 Flooring Replacement Phase 1 package. A Bid Bond IS required with submission of seed project #542-18-107 Flooring Replacement Phase 1 package.

QUESTIONS AND ANSWERS

1. MATOC Past Performance Questionnaires – Will projects with contract values greater than $5,500,000 be accepted?

A. Yes.

2. Seed Project Past Performance – Is the range of $100,000 to $600,000 for construction costs indicative of the flooring subcontract value or the overall project value?

A. The PP submitted for the Seed Project should reflect a standalone flooring project with a range of $100,000 to $600,000, or an overall project with a flooring subcontract value within the range of $100,000 to $600,000.

3. MATOC Offeror’s Past Performance – Do the submitted projects need to be 100% complete at the time the MATOC solicitation was released? 50% complete? Started?

A. See the solicitation page 86 of 147, 5.2 Section 2 – Past Performance, 5.2.1 Submission Requirements.

4. Will a PPQ Cover Letter template be provided?

A. No.

5. Who should be listed as the Project Owner on the Bid Bond for the seed project? Obviously the Department of Veterans Affairs would be named, but which address? Network Contracting Office 4 or Coatesville VA?

A. Coatesville will be listed for task orders accomplished at the Coatesville VAMC and Wilmington will be listed for task orders accomplished at the Wilmington VAMC.

6. Will scalable drawings of the work area be provided?

A. No.

7. Will another site visit be scheduled?

A. No.

8. When you have time could one of you please email me the slide presentation you ran this AM?

A. The slides are confusing and will not be offered. Refer to the solicitation.

9. Page 94 of 148 section 5.2.1 states VA requests one construction Flooring project that best illustrates contractors prime and subcontractors teams qualifications. Does this project have to be a single trade flooring contract or be part of a larger contract listed as 1 of the 5 past performance projects required on page 86. Please advise.

A. See answer to question number 2 above.

10. Page 86 of 148 section 5.2.1 references past experience to be $2,000-$5.5M within the last 5 years. However on page 94 of 148 section 5.2.1 states to show the team’s construction performance, magnitude between $100,000-$600,000 within the last 3 years. Is the requirement on page 96 referencing specifically to a flooring contract value of $100k-$600k or general experience. This would then contradict the requirement on page 86. Please advise/clarify the intent of the requirement on page 96.

A. Page 86 references the MATOC and page 96 references the seed project.

11. During the tour it was brought up that some of the arrows in the drawing may be shifted. What does that mean?

A. A corrected copy of the SOW will be posted to FBO.

12. We understand that drawings will be reposted via an amendment and assume that this problem will be alleviated?

A. Yes

13. What type of laminate is the VA interested in or would it be preferred that we make that recommendation?

A. Luxury Vinyl Plank INTERLOCKING measuring 6” x 48” each plank equal to or greater than: Philadelphia Commercial Resilient Collection IT’S A SNAP color to be close to a light oak and determined by the COR. Product must have a 10 year commercial warranty and a 3 year installation warranty.

14. Particularly in the area of the hospital where the flooring is buckling (building 1), has a moisture test been conducted? If not, will one be conducted? If there is a moisture problem, will SOW be modified to account for remediation of this problem?

A. No moisture tests have been conducted. Bid accordingly. The SOW mentions sealing with a 3 year installation warranty on page 9 of 10, paragraph 2.

15. Please provide the sign in sheet from the Site Visit A. The sign in sheet is included in this amendment.
