About this file
This is a Credential Request Form used by the 45th Security Forces Squadron (45 SFS) to process access credentials for Patrick Space Force Base (PSFB), Cape Canaveral Space Force Station (CCSFS), and Vandenberg Space Force Base (VSFB). The form is subject to the Privacy Act of 1974 and is authorized under Executive Order 9397 and Department of Defense Directive 8500.1 to determine applicant eligibility for installation access control in compliance with HSPD-12, DoD 5200.8, DTM 09-12, FIPS 201, and AFMAN 31-113.
The form collects mandatory visitor information including legal name, date of birth, Social Security Number, driver's license number, citizenship status, physical descriptors (gender, height, weight, eye color, hair color), and Common Access Card (CAC) status. Sponsoring organizations must complete the access dates/times section, specifying start and end dates, authorized hours of access for each day of the week, requestor name, agency/organization, and duty phone number. The form requires badging official signatures and allows for remarks regarding processing instructions. Processing timelines are three duty days for both CCSFS (submitted by hand delivery, fax to 321-853-3441, or email to 45sfs.s5v.ccafsvcc@us.af.mil) and PSFB (email to 45sfs.s5.vcc@spaceforce.mil). Applicants must acknowledge compliance with installation driving procedures and, for CCSFS access, review advisement of toxic chemical hazards used in launch vehicle processing. The 45 SFS conducts National Criminal Information Center (NCIC) checks to complete the credentialing process.
