About this file

This is a New York State Department of Taxation and Finance Contractor Certification form (ST-220-CA) related to a New York State Department of Transportation procurement for genuine OEM parts for Mack Truck, John Deere Equipment, and hydraulic supplies. The form serves as certification documentation that contractors must complete and submit to covered agencies for contracts subject to Tax Law Section 5-a. The procurement, identified as BA26-013, applies to contracts exceeding $100,000 in value for commodities or services where the solicitation began on or after January 1, 2005, and the contract was awarded, amended, extended, renewed, or assigned on or after April 26, 2006. Interested bidders are encouraged to submit inquiries to Purch-Assist@dot.ny.gov to request the Invitation for Bid package and be placed on the bidder's list. Bidders must provide their full business entity name, address, contact telephone number, and federal employer identification number when requesting solicitation materials.

The ST-220-CA form requires contractors to certify that they have filed Form ST-220-TD with the Department of Taxation and Finance or that they have previously filed such documentation in connection with another contract and that the information remains current and accurate. The certification must be executed by an authorized representative of the contractor and acknowledged before a notary public. Contractors must select one of two certification options: either confirming that Form ST-220-TD has been filed in connection with the current contract, or confirming that a previously filed ST-220-TD remains valid and accurate. The form includes fields for contractor information, covered agency details, contract number or description, estimated contract value, and notarized acknowledgment sections that vary based on the contractor's business structure (individual, corporation, partnership, or limited liability company).
