About this file
This document is a Data Item Description (DID) for Failed Item Analysis Reports (FIAR) for the LASSO (Low Altitude Stalking and Striking Ordnance) program. The FIAR is a detailed reporting requirement for documenting test failures during system and component-level testing, reliability testing, and government developmental/operational testing. Vendors must submit a comprehensive report within specific timeframes: initial notification within 1 business day of identifying a failure, and a final report within 10 business days, which must include root cause analysis, corrective actions, and potential impact on similar products or processes.
The report must contain specific detailed information including the failed subsystem's part and serial numbers, test failure description, operating hours at failure, affected units and lot numbers, a summary of failure analysis, measurements, root cause identification, proposed corrective actions, and validation of those actions. The document mandates that each failure requires a separate FIAR, though multiple failures with the same root cause may be combined with Integrated Product Team (IPT) agreement. All reports are subject to government review and must be formatted in Microsoft Office compatible documents, with distribution limited to Department of Defense and U.S. DoD Vendors under Distribution Statement D.

Remarks:
For each failure and Test Incident Report generated that occurs during testing, the Vendor shall perform root cause and corrective action analysis and submit a FIAR to the Government for acceptance.
Initial notification to the Government shall occur within 1 business day of the failure, which is defined as identification of a symptom versus a confirmed failure determined after engineering analysis.
Within 10 business days of failure occurrence, including those occurring during Government testing, the Vendor shall prepare and submit for concurrence the final FIAR. If applicable, the final FIAR shall also include the final Sub-vendor Corrective Action Report.
Government review/comments will be provided within 5 business days.
The Vendor shall incorporate corrections and resubmit a final report within 5 business days of receipt of Government comments.
The Vendor shall submit to the IPPT for concurrence, within time limits a final FIAR.
The final FIAR, shall include, if necessary, a
Sub-vendor Corrective Action Report (SCAR). E-mail is the preferred method of FIAR submittal.
The Vendor shall prepare a separate FIAR for each failure. The Vendor may prepare a combined FIAR for multiple failures of the same root cause if agreed to by the IPT. The FIAR shall not be considered closed until the IPT has concurred with the report.

Title Failed Item Analysis Report (FIAR)
Data Item Description (DID) Number
DI-RELI-80253
Description The FIAR documents test failures that occur during system, and major component-level testing, reliability testing, and Government DT/OT. The FIAR shall include identification and verification of the failure mode, a summary of the failure analysis and identification of the root cause(s), disposition of the failed item(s), action taken to eliminate the root cause(s) and to prevent recurrence, corrective action for all materiel affected including items that may have been delivered to the Government, verification/validation the corrective action was effective, history of the failure, containment of the failure, recommended disposition of the lot, and applicable Sub-vendor Corrective Action Report.

Preparation Instructions
The Vendor shall submit to the IPPT for concurrence, within time limits a final FIAR. The final FIAR, shall include, if necessary, a Sub-vendor Corrective Action Report (SCAR). E-mail is the preferred method of FIAR submittal.
The Vendor shall prepare a separate FIAR for each failure. The Vendor may prepare a combined FIAR for multiple failures of the same root cause if agreed to by the IPT.
Format Vendor format is acceptable; however, the IPPT at the Post Award Conference shall concur with the complete proposed content of the FIAR, and a list of personnel to be notified of failures. All subsequent FIARs shall adhere to the IPT-concurred format. The FIAR shall not be considered closed until the Government has concurred with the report.
