About this file
This is a Request for Statement of Qualifications (RFSQ) issued by the County of Santa Clara, California, Health System Behavioral Health Services Department (BHSD), seeking qualified firms to provide program evaluation services. The RFSQ operates through a two-phase process: Phase One establishes a Qualified Pool (QP) of Offerors across six service categories—Integrated Service Model, Mental Health Program, Substance Use Treatment Services, Quality Assurance and Data Analytics, Property Management Services, and Other Evaluation Services—while Phase Two involves issuing Work Order Proposal Requests (WOPRs) to QP members for specific projects. Responses for the initial round were due October 3, 2023, with subsequent response windows annually on March 1 through August 1 through May 1, 2028. The RFSQ remains open until September 1, 2028, with evaluations conducted at scheduled intervals. Pre-proposal conferences were held September 18 and 29, 2023. Initial round responses were evaluated by January 19, 2024, with qualification notices issued in February 2024. Offerors must submit responses electronically via email to BHSD.Solicitations@hhs.sccgov.org. Contract terms are five years or through the RFSQ close date, whichever occurs earlier, with the contract becoming effective upon execution by the County.
Offerors must submit comprehensive qualification packages including organizational structure documentation, evaluation criteria responses, customer references, declarations, cultural competence forms, and service rates. Responses are scored on Organizational Structure and Experience (75 points), Proposed Program Description (20 points), and Historical Performance Analysis (5 points), with a minimum threshold of 70 percent required to advance. The County may award multiple contracts and maintains sole discretion in awarding to any QP member based on future needs and available resources. Funding is contingent on appropriation by the County Board of Supervisors and may include County General Fund, federal, and state sources including Mental Health Services Act, Administration for Children and Families, and USDA funding. Offerors are responsible for meeting County insurance requirements, including certificates of insurance and additional insured endorsements. Medi-Cal reimbursable direct patient services cannot be subcontracted, though non-Medi-Cal services are similarly restricted from subcontracting under the RFSQ terms.

f. The County will continue to accept Offeror Responses throughout the RFSQ duration to qualify Offerors to the QP. In alignment with the established process, the County will continuously review and qualify additional Responses submitted by Offerors, as detailed in the evaluation schedule provided in Section II.A Table 3 of this RFSQ. • This RFSQ is contingent on the appropriation of sufficient future funding by the County for the services covered by this RFSQ. If funding is reduced or deleted for the purpose of this RFSQ, the County has the option to either cancel this RFSQ with no liability occurring to the County or to offer an addendum to the RFSQ that reflects the updated amount of funding.

In each fiscal year of an active Agreement, the total amount actually expended by County hereunder (“annual expenditures”) may not exceed amount allocated to the BHSD by the County Board of Supervisors and any other funding sources in the approved budgets.
