About this file
This is a Past Performance Reference Information Form template used to document and evaluate an offeror's relevant contract experience during the proposal evaluation process.
The form requires offerors to provide comprehensive information about reference contracts, including the offeror's name and UEI, the reference contract client (government agency or prime contractor), the entity that performed the work with its UEI, contract number and title, and client point of contact details (Contract/Subcontract Lead, Project Director, or Technical Representative). Offerors must document the contract period of performance, contract type (such as Fixed-Price, Time and Materials, Cost Reimbursement, IDIQ, BPA, or BOA), total contract value with average annual value, and value performed to date as of the RFP release date. The form emphasizes complexity assessment, requiring offerors to summarize the scope, work difficulty, similarity to the current solicitation, problems encountered, and creative solutions implemented. A critical component addresses safety performance, requiring offerors to demonstrate commitment to safe work practices through documentation of their safety program, relevant policies, and performance metrics including Experience Modification Rates (EMR) for the past three years where applicable, with explanation required for any EMR exceeding 1.0. Offerors may also disclose relevant regulatory citations from OSHA, Cal-OSHA, or equivalent authorities from the past three years with high-level summaries of citation nature and resolution status. The form is limited to 3-5 pages per reference contract, though offerors may amend the format while maintaining specified font, size, and page requirements. Failure to provide safety information will not automatically render a proposal non-responsive, but the evaluating organization reserves the right to request clarification on the offeror's ability to perform work safely and responsibly.
